Patient journey modelling

I am proud to present our publication 'Patient Journey Modeling in CSU'. This holistic approach covers the trajectory from the patient's perspective and incorporates the insights of the healthcare providers in a co-creation approach. Based on real-world data, we were able to create a consensual model that can be used to simulate what-if scenarios thus identifing hurdles and other barriers. This enables open communication about the impact of campaigns on individual patients, but also on the patient population, both over time and quantitatively.

This is the first step towards optimizing patient care and at the same time initiating a resource-saving way of thinking the future healthcare system.
