Over 7,000 patients at Haughton Thornley Medical Centres now have access to their FULL GP electronic health records and understanding too

Today we are pleased to announce a number of milestones we have reached including 7009 patients (57% of our total patient population) having full access to their GP electronic health records, over 4000 patients at Thornley House Medical Centre, over 2000 patients with low back pain (67% of all such patients), over 900 Bengali patients (59% of all such patients), over 1500 patients with anxiety or depression (94% of all such patients) and 40% of our patients with Learning Disability. This is a fantastic testamount to all our patients and staff who have helped us achieve this. The practice-based web portal www.htmc.co.uk helps to keep patients informed by providing trusted information that they can easily access from whereever they are anywhere in the world at a time of their choosing.

Whilst many people think all that is needed is an app and it will just happen, the truth is far from it. I hope to share some of our experiences on here and bring out some of the vast resources we have developed to support our patients and staff and how I use them in the consulting room to help others understand too. My purpose is simple - I am keen for other GP practices to want to do similar things - not just offering patient access but undertanding too. This is truly a fantastic achievement. If your patients want to upload their record to a private provider, can that be done at a press of a button?

Silos are a big problem. Understandable why these are created when vendors need to see a return on their investment until we realise it is the tax payer who is paying for all this. If the record also linked to trusted info from the hospital website then that may encourage me and the patient to look at it too in order to learn more #RecordsAccessAndUnderstanding. Having a common consent process would greatly help. Currently 59% of our cancer patients have full accesslectronic Health Record https://www.htmc.co.uk/pages/pv.asp?p=htmc0328. They all should be able tuse you never know when you might need to use it! Even after midnight now!

Thanks David Geran You raise a very important issue - what are we trying to do and for who. I never talk about patient-centred care.... instead preferring to talk about the #PartnershipOfTrust which is about a relationship between a patient and clinician usually. Healthcare is a team sport - rarely done if ever by just one person - whether that be the patient or the doctor / nurse other. We have had access to the Christie GP portal ever since its inception thanks to Rhidian Bramley. It has been useful for MDT meetings in Gold Standards Frameworks meetings that Lisa Gutteridge uses. I cannot say I have ever needed to log in as a "regular GP" although it would be great if patients could see what the clinician sees.

We cannot be held responsible for what others do. However we can share our own experiences and knowledge and describe what we do.It is for others to consider what this means for them. Ultimately my job is to help people and support them to learn a little more about what they can do to help themselves and for me to learn a little more about them. We both learn more about each other. She now has a link to the practice-based web portal www.htmc.co.uk so that she can learn even more and go beyond what we wrote in the electronic health record. We are very fortunate to have patients like her who keep shining a light on what they can do if we ask the right questions and reach out to work together. As others begin to do the same or similar, they too will start to feel the same and reignite why we went into medicine - to serve others and to make a difference in their lives too. This is not easy and we may not win prizes or build our bank account. However the thanks we get is priceless and energizes us for the next patient and the next challenge we face. Imagine what could happen if each of began to do the same?

This week I saw a lovely elderly lady who came to see me. When I asked her if she had access to the internet, she paused and then said "Yes, I have a kindle". I said "Great". Please bring it with you when I see you and we will go through the consent process together and then I will set it all up for you in the consulting room. She was very pleased. After we completed the medical consultation, I asked her if she had brought her kindle with her. She said "yes" and the opened her bag and brought it out. She didn't have a 3G / 4G connection so I opened up my hotspot using my phone and then proceeded to complete the online questionnaire with her and then set up her kindle to view her records. She was very, very pleased. Something like Summer, Autumn, Winter and Spring rolled into one. Our Values are expressed by what we do particularly with those less fortunate than us or who cannot "repay" us. Actually her smile said it all. She is a swallow that tells me summer is here!
