Output V/s Outcome

In my experience of interacting with multiple people from business backgrounds, I have seen a sizable number like to use management jargons in their discussions quite frequently.
It is great that you have the know-how of something and you wish to bring it in or showcase it during an interaction with another person. However, many times certain jargon or words are not understood well and many tend to use it inter changeably with another jargon or term. This may seem like an innocent mistake however; I have seen it become a recipe for disaster many times.
One such term is Output which is many times used inter changeably with Outcome. When business leaders make this mistake, they inadvertently throw off the productivity of their team and the chance to achieve their set goals and targets.
This confusion occurs mainly because many believe that the result of an activity is the outcome. This is incorrect. The result of an activity is output and the effectiveness of the same is outcome. In business, all our goals and targets are dependent on the effectiveness of the activity. Hence, when we are output focused, achieving goals would be a hit or miss, completely dependent on luck and fate. However, when we move our focus from output to outcome (ie effectiveness of output), we are sure to achieve the targets.
