Home Modification Process

Step 1: Estimates for Modifications

For any home modification exceeding $1,000, we require three estimates. OneWell Health Care provides one of these estimates, while the family is responsible for obtaining the other two. This process ensures transparency and allows for competitive pricing.

Step 2: Medical Necessity Documentation

A key requirement for modifications is proof of medical necessity. This can be fulfilled by either a letter from the participant’s primary care physician (PCP) or an Occupational Therapy assessment. Along with the three estimates, this documentation is essential when the Supports Coordinator submits the modification request to the Office of Developmental Programs (ODP) for review.

Step 3: Implementation and Reimbursement

If OneWell Health Care is selected to complete the modification, we handle everything from supplying materials to providing labor. After the project is completed, we submit the necessary documentation to the ODP for reimbursement.
