Get a password manager. 1Password, Dashlane, LastPass, KeePass, RoboForm, just pick one and use it. With a password manager, you create a unique password for each website and the password manager remembers them for you.

If a site has security questions, make up fake answers (a breach on one site may reveal your mother's maiden name or the street you grew up on.) When prompted with a security question, copy the answer from the password manager.
