Nutritional Wellness Assessment

Diet plays a central role in human health and well-being. The body's metabolism is also determined by the diet. Each of the human body's organs and tissues needs all the nutrients, vitamins and minerals to function correctly. However, this remains a challenge as many people cannot follow the right dietetic measures, leading to malnutrition and, therefore, diseases. This nutrition assessment enables one to consider what a person tends to take in and where he/she has to transform.

Nutrition Assessment

To complete the nutrition assessment, please answer the following questions honestly and to the best of your ability. You can use the scoring system provided for each question to indicate how often you engage in the specified behaviour.

  • Almost always = 2 points
  • Sometimes/occasionally = 1 point
  • Very seldom = 0 points

Total Score

Add up your points to determine your overall nutrition score. Use the scoring guide below to interpret your results:

  • 15-20 points: Excellent nutrition habits. You are well on your way to maintaining a healthy diet.
  • 9-14 points: Needs improvement. Could you identify areas where you can make adjustments to improve your nutrition?
  • 0-8 points: This needs significant work. Start by making minor changes to your diet and gradually work towards a healthier lifestyle.

The nutrition assessment is one of the most helpful tools for analysing your dietary behaviour and recognising necessary changes. The results suggest that this assessment can help one learn about the nutrients he/she requires in his body and diet. It is always important to note that even those changes cumulatively affect overall health, and any positive change is a good one.
