This nurse helps fulfil her patients’ (last) wishes

"Always be contented, patient and forgiving."

Meet Hay Mahn Teresa, an Advanced Practice Nurse in gynaecological oncology. From diagnosis to palliative care, Teresa is part of a care team that offers personalised treatment plans to patients. While a lot of her work is centred on clinical care, what she does beyond the clinic makes an equally lasting impact.

Q: Thoughts on Advance Care Planning (ACP)?

Individuals can plan for their terminal life stage by using ACP to communicate medical care wishes, preferences and goals to their loved ones and the healthcare team. It is not just about paperwork, it is a gift to their loved ones, ensuring the family understands and honours the individual’s wishes.

Q: Share more about your role in the final life stage of a domestic worker who had advanced cancer?

The Burmese patient was in the intensive care unit, requiring oxygen and medication to support her blood pressure. The palliative care team was called to manage her symptoms and discuss her ACP. Her only wish was to see her two sons and elderly mother back in Myanmar as she had not visited them for 2 years. Time was running out and the situation seemed impossible. I felt helpless but encouraged her to be emotionally strong. I also updated her mother over video calls. With the generosity of JC Trust and coordinated effort from Hope Emergency Ambulance Service and 新加坡航空 , our dedicated multidisciplinary team was able to arrange for her to fly home when her condition was deemed stable. She was able to spend time with her family in her hometown before she passed on. I was inspired by her strength and perseverance to see her family despite the unpredictable circumstances she faced.

Q: Tell us more about how the wedding for a 52-year-old patient was fulfilled in the ward?

In 2022, our team reached out to Ambulance Wish Singapore (AWS) — a charity that supports terminally ill patients by creating positive end-of-life experiences. In this case, our patient's last wish was for a wedding ceremony to take place in the ward. AWS facilitated the arrangements including make-up and photography services. I was among the other patients, their loved ones and nurses who witnessed the joyous and touching moments of connection, amidst laughter and compassion.

Q: What do you do in your free time?

I enjoy spending time with my family — taking strolls together, chatting, and going on vacations. I also ensure that I have some “me” time occasionally, with Pilates, Barre, shopping and Netflix.

Q: A message you would like to share with your patients?

You are not alone. We stand with you, walk beside you, and support you every step of the way. So, “You’ve Got This!” Each day is a step forward, your courage inspires those around you, and you are always cherished by the healthcare team for who you are.
