QUIZ ON NATIONAL SAFETY DAY TO ENRICH SAFETY KNOWLEDGE

1.0 Preamble

As we, all know during & after Covid-19 pandemic situation many industrial disasters have occurred causing damage to Human, Environment & Facility in India. Human Factor is the most important in avoiding / preventing such disasters by empowering his/her knowledge in work place & Process safety. To upgrade the knowledge of personnel working for the industry, we are bringing safety quiz on various topics including NSC Theme of the year 2021 (Learn from Disaster and prepare for a Safer Future).

2.0 Rules of the online quiz

1. The quiz is accessible online on the following link: www.nexapsm.com
2. Quiz will start on  4th March 2021 at 4: 00 AM to 5th March 4:00 AM
3. The quiz comprised of multiple-choice questionnaire (50 Numbers MCQs) randomly picked from question bank database on

Each question having one correct answer from the options.

1. Quiz is time bound, each question will have 30 seconds only and total duration of 25 minutes for quiz completion.
2. Participant can take competition quiz maximum 3 times during competition and after competition (5th March 2021, 4 : 00 AM afterwards) as many times as he/she wishes to take.
3. First 100 rankers will be declared on our website on 5th Mar 2021 at 9:00 Hrs in download section. Top 6 winners list will be on our website Home page.
4. Log in can be done on our website by entering following information.

• Full name
• Company and designation
• Personal Email ID (Must be valid to get online certificate)
• Contact Mobile No

1. On submission it is assumed that you abide with all terms and conditions of Quiz and Quiz will start.
2. Various other quizzes are already available on our website in Quiz Zone.

3.0 Terms & Conditions of participation

1. Registration Information is mandatory for participation & communication.
2. Online certificate will be generated as per name registered during participation and we cannot modify or issue certificate as it’s system generated.
3. Participation in the quiz is free and open to all persons. Any person can take part in the quiz who is working with process industry of any category. There is no age limit or experience requirement for the participation.
4. The persons who have given incomplete or inaccurate personal/ contact details will be disqualified for the competition quiz.
5. Participation in the quiz implies that participants accept the integrity of these rules. Non-compliance with the rules results in the automatic cancellation of the participation in the quiz and of the candidacy for the prize.
6. Nexa Process Safety Engineers and their family’s members including relatives cannot take part in this competition.
7. Nexa Process Safety Engineer’s responsibility will not be engaged if, in a case of force majeure as recognized by the local laws, or any event beyond its control to cancel the quiz.

4.0 Awarding the prize & Description of the prize

1. First 6 winners will receive online Amazon Coupon in email id provided at registration with mobile number. Ensure email is working for such delivery. The winners will be contacted by e-mail & through mobile number. The winners have a month to accept their prize.
2. 1st Prize : INR 2000 Amazon Gift Card
3. 2nd Prize : INR 1500 Amazon Gift Card
4. 3rd Prize : INR 1000 Amazon Gift Card
5. 4th Prize : INR 500 Amazon Gift Card ( Consolation Prize )
6. 5th Prize : INR 500 Amazon Gift Card ( Consolation Prize )
7. 6th Prize : INR 500 Amazon Gift Card ( Consolation Prize )
8. Prize is fixed and cannot be transferable or exchangeable. It must be redeemed within a month. In case of non-redemption of the card it will be taken back by Nexa PSE.
9. Winners are defined as who score the maximum in minimum time for the quiz.
10. Winners will be defined by system and no human intervention at our end.
11. Those who will score 70 % can immediately download the certificate at quiz end and also receive the same on registered email id. Lower score will not get the same.
12. Winners Photos will be displayed on our site if received immediately on declaration of result and our communication.
13. Company can promote their employees to take the quiz as a part of safety week celebration and higher scorer in your organization can be awarded with suitable prize.

5.0 Confidentiality, Dispute and Responsibilities

1. Nexa Process Safety Engineers assumes no liability in case the internet network, server, logics, mobile phones, or any reception tool dysfunctions and prevents the effective functioning of the quiz. Moreover; its responsibility will not be engaged for problems related to log In issues, reception or answering or e-mail.
2. Nexa Process Safety Engineers assumes no liability in case of technical, material or software malfunction or defect (virus, bug) affecting the participants’ computer system or computer equipment and data, and the related consequences on their professional, personal or commercial activity.
3. Nexa Process Safety Engineers will be able to, at any point in time, notably for technical reasons or for updates and maintenance, interrupt access to its website and the quiz it contains. Nexa Process Safety Engineers will in no case be responsible for these interruptions and their consequences. No compensation can be claimed on these grounds.
4. The answers given by participants will be kept confidential and will not be revealed to anybody, even asked for by anybody. The data base will be deleted after 7 days of declaration of result.
5. The data contained in the quiz is strictly covered by professional secrecy, and this also applies to all information communicated to Nexa Process Safety Engineers during the quiz. Nexa Process Safety Engineers commits to taking all necessary protective measures in order to keep this information secure and notably to prevent their alteration, damage, or their being transmitted to unauthorized persons.
