Did you know that many emergency department presentations or hospital admissions are potentially preventable if people have timely access to appropriate healthcare services? Excessive transfers of care recipients to emergency departments may indicate poor care quality and access.
Residential care providers must collect and report quarterly data on hospitalisation, including the percentage of care recipients who had one or more emergency department presentations.
This helps ensure that timely and appropriate healthcare services are provided to minimise hospital visits.
Key Terms: An emergency department presentation occurs when a care recipient presents to an emergency department or urgent care centre. A hospital admission occurs when a care recipient is accepted by a hospital inpatient specialty service for ongoing management, whether planned or unplanned.
Assessments and Measurements: Care providers must assess and record hospitalisation data for all care recipients residing at the service during the quarter, excluding those who were absent for the entire quarter. They must record the number of care recipients with one or more emergency department presentations or hospital admissions during the quarter.
Data Reporting: Providers must compile and report this data without including any personal information about care recipients. This information is crucial for understanding the quality of residential care and identifying areas for improvement.
By monitoring and reporting on hospitalisations, residential care providers can identify areas for improvement, ensuring better care quality, and reducing unnecessary hospital visits for care recipients.
