Transformation is not the responsibility of IT teams. It's a business issue. Transformation is about People and Processes; the tech is an enabler. Business leaders need to consider the required business outcomes, then the process and people elements, and then any tech changes needed to support the people and processes. Many transformations fail, and in many cases it's because it's seen as a tech change and little else. Then businesses wonder why they aren't realising the expected benefits.
