MRI changes of presumed cervical spine GMA.

Hello again Dear Colleagues; this time I wanted to share this MRI case I saw at the Vet College / University of Saskatoon. Dog was presented to the hospital due to acute onset of neck pain according to the owner. So first differential considered was acute IVD extrusion. Therefore MRI of the cervical spine was performed. It was really surprising to see that all the intervertebral disks showed fairly good signs of hydration (hyperintense on T2) and in normal anatomic location, no spinal cord compression. What was really interested was the wispy faint hyperintense areas seen diffusely distributed in the spinal cord. Images in T1 were unremarkable and the T2 hyperintense lesions showed mild increased signal intensity in T1 post gadolinum contrast images indicating some uptake of the contrast medium. An inflammatory process was immediately thought as first differentials, GMA was number 1 in the list.   CSF analysis was recommended and result was an inflammatory component highly suggestive of GME. Therfore no surgery this time.
