4 天前Thanks, Ben Hutchinson, for another great share.?Unfortunately, the objective of most investigations is to 'identify root causes to prevent recurrence'.?(Arguably almost impossible.) They are not looking to reduce risk. This drives linear thinking and not the exploration of uncertainty.?Nothing will improve until organisations change what they want an investigation to achieve.
