Medicare has expanded 4-year health history FHIR data-sharing to healthcare providers with a patient treatment-related purpose with no requirement of participation in an Accountable Care Organization (ACO) or other value-based care program. Medicaid and Medicare Advantage health history will be released by July 2021. 

With this widely available data source, medication reconciliations will now take a fraction of the time as all data from all providers will be instantly presented at the point of care in the EHR. Healthcare providers will no longer need to rely solely on a patient's recollection of their medication names and related diagnoses. In addition, patients will not have to deal with frustrating paper forms.  A reduction in medication errors is also anticipated as a view of a comprehensive history will be available to all providers involved in a patient's care.

In addition, a healthcare provider will be able to see if a patient fails to pick up a prescribed medication and implement the important next steps of medication adherence with the patient much more quickly.
