West Chester PA. December 11, 2024. Maternal and Child Health Consortium (MCHC) is excited to announce it has received a $50,000 grant to help fund supportive services for low-income parents in the Chester County region. The Foxwynd Foundation Donor Advised Fund (DAF) funded the Foxwynd Foundation’s generous donation to MCHC.

Grant funds help MCHC to address disparities in health care access for pregnant individuals and promote K-12 readiness by providing comprehensive home-based support for pregnant individuals and families with children up to the age of five leveraging the evidence-based Parents as Teachers model.
MCHC provides free services to participants, including group connection events, assistance with enrolling in health insurance and food benefits programs, and specialized support for families involved in the child welfare system. As a vital resource for families with young children, MCHC is committed to strengthening communities and supporting families in need.

Personalized Health Education: Home visitors provide one-on-one guidance on a variety of health topics, including maternal and child health, nutrition, breastfeeding, and infant care, tailored to the family's specific needs.
Prevention and Education: Home visitors play a key role in preventive care by offering education on topics such as immunizations, safe sleep practices, and injury prevention.
Early Intervention and Developmental Support: Home visits help parents recognize and address family stressors and health concerns early, ensuring that children reach important milestones in time for school and receive the support they need.
