Mapping Self-Service Variables to Incidents in ServiceNow

We had a request today to add a new option to a variable on our self service page. That request also included setting or mapping certain fields in the resulting incident record to specific values. Here was our solution. I would appreciate any feedback if you think there are better (best practices) ways to accomplish this.
