A Look Back to 2020 with Cleveland Clinic- Euclid Skilled Nursing Facility

It is hard to look back to 2020 - a time most of us would like to soon forget. It was a time when healthcare employees were more in tune than ever with their company's culture and it became clear to strong, connected teams that the only way to get through the storm was together.?

One unit that was called to action and rose to the occasion was the Skilled Nursing Facility (SNF) at Cleveland Clinic - Euclid hospital.?If you can recall, nursing homes were one of the first hit hard by the Covid-19 pandemic. Nursing homes all across the country were at a loss of what to do, and fear of the unknown caused many facilities to just accept these unfortunate care disruptors as another symptom of the pandemic.??

Thankfully, Cleveland Clinic- Euclid's SNF could not ignore this huge gap in care for our most vulnerable population. They saw the need for a covid positive unit within their community and accepted the challenge. As reported in the Euclid Observer, from April 2020 through April 2021, up to 80% of Euclid’s SNF beds were converted to care for patients recovering from COVID-19. Patients who needed additional care to safely return home were able to recuperate in one of the SNF’s private rooms.??

This resulted in the unit being awarded a Five-Star Quality Rating from the Centers for Medicare & Medicare Services (CMS), based on 2020 data.?Some highlights include:?

• Shorter length of stay than most SNFs (Euclid SNF, 10 to 14 days; other SNFs, average 20+ days).
• Successful return home (Euclid SNF, 77.8%; Ohio average, 54%).
• Reduced cost of care.?
• Only 3% of Euclid SNF patients visited the Emergency Department within 30 days of discharge.

It goes without saying that this is a proud team, proud of what they were able to do for their community and proud they were able to come together in such a challenging time.?
