Assertive Community Treatment

Millions of people across the United States struggle with mental health challenges. In previous years, nearly one in five Americans were reportedly living with a mental health condition. Recent data from the Centers for Disease Control and Prevention indicates that this year, one in four adults will experience symptoms of anxiety or depression.

Assertive Community Treatment (ACT) is a full-spectrum, long-term and community-based treatment model for adults with severe and persistent mental health challenges that have not been adequately treated by more traditional services. This evidence-based treatment approach involves a high level of coordinated care and supports individuals as they manage their mental health challenges and live independently.

A "Person-centered plan" and "Personal Outcomes" are the core of ACT. This means...

  • Fewer hospitalizations
  • Access to stable housing
  • Effective medication management
  • Progress in client goals
  • Improved support systems

Our unique approach involves a team of highly-trained Easterseals UCP staff who collaborate to assess participants’ needs and develop a plan to connect them with the services they need to thrive. In some cases, we are able to provide direct services through our mental health and employment programs.
