Kudos to Critical Care Hope E-ICU & Critical Care Team Bikaner for saving the life of a 25-year-old female patient from a life-threatening condition

Critical Care Hope Tele ICU & Apex Hospital Bikaner added another feather to their cap by successfully treating 25-year-old female patient Mrs. Pooja who was just near death when brought to Apex hospital, Bikaner on 20 Feb 2023 night in a very critical condition.


The patient was referred from another hospital as she had consumed a pesticide (organophosphorus compound) 5 days ago. At the time of arrival, she suffered cardiorespiratory arrest, and was unconscious, intubated, and on ventilator support. Upon admission in the Medical ICU, the treatment was started under the guidance of Dr. R.P. Agarwal.


The patient’s oxygen requirement was high and a chest x-ray revealed infiltrates in the right lower zone. She was started on broad-spectrum antibiotics and atropine infusion apart from other supportive treatments. Critical Care HOPE Command Centre played a crucial role here as the ventilator management and cardiac monitoring were strictly supervised by it.


Within the next 36 hrs, she became conscious and was extubated. But the next day itself, she had respiratory problems due to muscle weakness and lung infection. She was again intubated and taken on a ventilator. Thereafter, a series of complications arose including development of septic shock and multi organ dysfunction due to pneumonia. She was aggressively managed and stabilized over the next 5 days. Tracheostomy was done on 28 Feb and the weaning process was initiated due to muscle weakness(intermediate syndrome).


The complications once again increased and unfortunately, she developed B/L pneumothorax and s/c emphysema on 2 March which was immediately identified and managed effectively. She was successfully weaned from the ventilator over the next five days and the tracheostomy was removed on 10 March.


According to the doctors working on this case, it is one of its kind of case where they saw a series of multiple complications (extubation failure, infection with multi organ dysfunction, barotrauma, muscle weakness leading to prolonged weaning, DVT).


24*7 vigilance of Critical Care Hope E-ICU and excellent work by the ICU team at Apex Hospital Bikaner under the leadership of Dr. Rupinder Kaur helped in successfully managing the case and it emerged as the life savior for the patient.
