"It's all about the bone". I probably say this statement to patients no less that 87 times each and every week. I'm sure I will say it again, starting this Monday morning. Most patients don't comprehend everything about dental implants, but they all completely understand this one simple statement. Why? Because it's true and makes perfect sense. This isn't brain surgery. When it comes to dental implants, if there is good bone quality and quantity, we are off and running. If we do not have good native bone, it completely nullifies the entire implant treatment objective and pushes the doctor towards alternative (mostly inferior) treatment options. Backs us into a corner. Game over. As they say in Spanish, "No mas implantes dental". Simply put, available bone foundations dictate the initial course of action and a lack of viable, quality bone totally wrecks everything.

So, why am I preaching so much about regenerating bone grafting? Because the vast majority of tooth extractions done today in dental offices all over the country WILL NOT have bone graft or even bone plugs placed in the socket! Amazing, yet quite appalling. Simple. Cost effective. Indispensable. Still, not even offered to the patient for a variety of reasons. This is more than a little problematic and in reality, could be considered slightly subpar oral surgery in many instances. When extraction sockets are not grafted, the potential for catastrophic bone atrophy skyrockets dramatically. Most existing dental implant bone grafting procedures are too complex, too unpredictable, and use particulate bone harvested from cadavers. These old school methods are very good grafting procedures overall, but most G.P.s have limited experience in actually delivering such complex and expensive treatments. Still to this day, they don't teach this stuff in dental school. Again, amazing, yet quite appalling. Every day, we have patients that present to our office searching for dental implant solutions, but they do not have the option because there is no bone. Sinus lifts, lateral ridge grafts, and other complex grafting procedures are first required before implants can be considered and these surgeries aren't cheap or fun for anyone. Smart socket preservation techniques grafts done early in the process can and should prevent these bad outcomes. Again, a win-win for everyone.
Without proper yet simple protocols that can be implemented easily and immediately, doctors severely limit patients' future implant options. The end result is the patient may not be able to have an implant or implants in the future due to a lack of bone volume. That's a devastating outcome for everyone involved.
