INTRODUCING A NEW COST EFFECTIVE FRAMEWORK: High performance caregiving

A New COST EFFECTIVE Framework for Managing Dementia and Memory Loss Care that Makes Existing Models Obsolete

Upon the death of her husband on June 19, 2020 during the coronavirus pandemic, the timing created a unique opportunity for Dr. éthelle G. Lord to grieve and to begin to reevaluate her work goals with the International Caregivers Association (ICA). It was a rare opportunity to contemplate a new direction for the ICA.

Dr. Lord decided to revisit the schema of the Care Provider Partnership Agreement Program (CPPAP), a program of care that focuses on improving upon existing models of long-term dementia and memory loss care programs for home care, assisted living, nursing homes, hospice services, and hospitals. With over 55 million confirmed global cases of dementia (even more when considering the unconfirmed cases), it is time to create a new framework of dementia and memory loss care.(1)

The Care Provider Partnership Agreement Program (CPPAP) is a holistic assessment solution for chronic problems endemic to providing care for aging care recipients often experiencing the progressive stages of dementia. This innovative program underscores the feasibility of rendering suggested care solutions in homebound and long-term care environments. While receiving care from caregivers under the guidance of interdisciplinary coaches, CPAP introduces workable care solutions with effective disease mitigation results for aging care recipients diagnosed with dementia worldwide.(2)

Statistics show that as many as two-thirds of residents in long-term care are experiencing dementia and memory loss.(3) In the years prior to the rise in dementia cases in assisted living facilities and nursing homes, care organizations were struggling to attract and keep a competent and dedicated workforce. The work of dementia care is not only demanding, it is a specialty for which most employees are undertrained to provide specialized care safely and adequately. In his latest book entitled Understanding and Presenting Harmful Interactions Between Residents with Dementia, Eilon Caspi stated “underrecognized and largely unaddressed phenomenon of distressing and harmful resident-to-resident interactions” create life and death scenarios (Pub. by Health Professions Press, 2022, p. 1). The coronavirus pandemic exacerbated adequate dementia care due to the lack of accessibility to residential settings by families and visitors, as well as staffing shortages. Unfortunately, many organizations closed their doors due to the contagious nature of the virus and insurmountable lack of competent staffing. Organizations that have remained open are struggling to stay afloat in this tumultuous economy. ?

Dr. Lord established her dementia-care consulting practice in Maine and California. The 24/7 care of someone living with dementia and memory loss typically begins in the home (home care). Since the caregiving fatigue timeline dictates that within 4 to 5 years family members become exhausted and can no longer provide such care, the individual living with dementia and memory loss must often be placed in long-term care, whether in a private home care-based service, assisted living, or a nursing home facility.??

Ethelle I am so glad you are doing this. The real problem is the people making the decisions have never been on the front lines. Boots on the ground workers NEVER get heard. We are never even consulted on what we need to do our job to the best of our abilities. We are told what the money will provide and we make due. Seriously my motto became “ we’re going to do the best we can with what we have”. The truth is it takes more money. Those profiting from the care are not willing to take a pay cut to make it happen. Oftentimes when things are cut from the bottom line, it’s usually the CNA/ HCA ‘s and the Clients that are the bottom line. Make a change where it counts. Client to CNA ratio’s. We have training but how can the Client get or CNA give the care they want to give and the clients so desperately need, when you have 9-12 clients per CNA /HCA. Because hiring more staff means less profit for the company. The frustration of it all is what sparks the abuse, I’m certain of it. P.s. because of the abuse I witnessed in Facility Long Term Care during my practicum I vowed to never work in a LTC facility. I have been blessed to work in Home Care. There is where I can give my dementia clients the one on one care.
