Innovating to Improve Care for People Dually Eligible for Medicare and Medicaid
Fourteen Safety Net Health Plans participate in the Financial Alignment Demonstrations from CMS which aim to better integrate care and coordinate benefits who are eligible for both Medicare and Medicaid benefits. Collectively, these ACAP-member Medicare-Medicaid plans (MMP) account for about 30 percent of enrollment in demonstration programs nationwide.
A new report from ACAP and CHCS highlights innovations health plans have implemented to fulfilling the promise of integrated care across the Medicare and Medicaid program: telemedicine initiatives to improve access, crisis centers, efforts to address unmet behavioral health needs, and more.
There has to be a better way for dually eligible beneficiaries to get comprehensive care than dueling fee-for-service systems. Safety Net Health Plans have made real innovations to deliver top-notch care and improve access. These are the kinds of innovations that can take place on an integrated care platform – they are far harder to realize in a fee-for-service system.
Take a look at the report here and let me know what you think.
