Immediate Replacement conform Groenendijk et al. results in minimal discomfort and the highest and most predictable esthetic outcome. In comparison with the conventional method the treatment period is diminished from 12-24 months to 6 months, takes only 1 minimal invasive flapless surgery instead of 3 to 5 extensive surgeries and has a more predictable and esthetic outcome. Instead of wearing a loose flipper during the healing period with immediate replacement directly a temporary fixed crown is made. In the conventional method a tooth is extracted and after 2 to 6 months implant treatment is started, however 32-69 % bone volume is lost within the first 3 to 6 months after extraction. Following the conventional protocol in 80 percent of the cases there is a lack of bone for implant placement. Then two or more extra surgeries are indicated to reconstruct the gum and underlying bone, with less predictable esthetic outcome and higher costs. The innovative and scientific based Immediate Replacement Protocol described by Groenendijk et al. is the preferred solution in case a tooth is considered lost. Do not let extract the tooth before a three dimensional scan is made to evaluate if immediate replacement is possible.
