Imagine this: It’s 2 a.m., and you’re lying awake, overwhelmed by a relentless stream of thoughts. Therapy feels out of reach—too expensive, too slow, too daunting. So, like millions of others, you download a mental health app, hoping for relief. For a week, you diligently log your mood, complete the exercises, and chat with the chatbot that promises it’s “always here for you.” But then life gets busy. The notifications feel more like nagging than help. And soon, the app joins the pile of forgotten icons on your phone’s screen.
Here’s the kicker: 96% of people abandon mental health apps within two weeks. That’s not just a footnote in a research paper; it’s a glaring indictment of how these tools fail to engage with real, messy human needs.
Promises Without Proof
The DHMI market is a behemoth. Thousands of apps claim to treat depression, manage PTSD, or improve mindfulness. Some do so responsibly, with evidence-backed methods. But far too many make bold, unverified claims while skirting regulatory scrutiny.
Here’s how: The FDA only oversees digital tools classified as medical devices. To dodge this, many apps label themselves as “wellness products,” sidestepping requirements for rigorous testing. This loophole allows companies to flood the market with tools that haven’t been proven effective.
The consequences are troubling:
Unexpected Truth: Digital Tools Aren’t So New
DHMIs look innovative on the surface: sleek designs, millions of downloads, billions of dollars in funding. But peel back the layers, and they often replicate old mental health frameworks. Digital versions of mindfulness exercises, mood trackers, or cognitive-behavioral therapy (CBT) tools may seem cutting-edge, but they rarely offer more than what paper workbooks and group therapy have for decades.
If you think digital mental health is cutting-edge, here’s a surprise: Many of these tools are just digital versions of practices that have existed for decades. Mood tracking? That’s been around since the days of paper journals. Cognitive-behavioral therapy exercises? Therapists have handed out worksheets for years. The difference? Now they’re on your phone, and companies are profiting from the packaging.
In fact, I co-authored and engineered one of the first "cybertherapy" digital mental health tools in the early 90s, before smartphones, apps, and Google even existed. It offered a rules-based psychoeducational and therapeutic support function and CBT exercises remarkably similar to today’s apps (but requiring distribution on a floppy disk if you can believe. We've come a long way, but still lacking in many ways.
Instead of disrupting mental health care, most DHMIs cater to a narrow audience while ignoring systemic barriers:
Generative AI: A False Saviour
Generative AI has entered the DHMI conversation, promising personalized, 24/7 support. At first glance, it seems revolutionary. AI chatbots can simulate empathy, offer tailored advice, and scale infinitely. But here’s the hard truth: AI doesn’t fix the deeper flaws of DHMIs—it magnifies them.
1. Surface-Level Personalization
AI chatbots can offer encouragement like, “You’re strong; you’ll get through this.” But can they recognize the subtle warning signs of a mental health crisis? Can they offer nuanced support for someone navigating trauma, systemic oppression, or cultural stigma? Not yet—and maybe not ever.
2. Ethical Hazards
AI lacks the ethical guardrails humans bring. A chatbot might dismiss suicidal ideation or fail to recognize when someone needs immediate, professional intervention. These aren’t technical bugs—they’re life-threatening oversights.
3. Bias at Scale
AI systems inherit the biases of the data they’re trained on. Chatbots trained on predominantly Western datasets may ignore the realities of users from collectivist cultures, marginalized groups, or underserved communities. Instead of democratizing care, they risk reinforcing inequities.
Breaking Free from the Illusion
To realize the promise of DHMIs, we need to stop chasing scale and start addressing substance. Real innovation won’t come from more chatbots or better interfaces—it will come from tackling systemic barriers and engaging deeply with the complexities of mental health.

4. Demand Accountability
Stop measuring success by downloads or retention. Measure it by outcomes. Are people feeling better? Is their mental health improving? Transparent, longitudinal studies are critical to build trust.
5. Regulate for Safety
Governments must close the regulatory loopholes that allow apps to avoid scrutiny. Every app claiming to “treat” mental health should meet the same rigorous standards as medical devices.
Our Call to Action
The illusion of innovation is seductive. It lets us celebrate progress that hasn’t been made and risks leaving those who need help most further behind. But we can demand better.
The future of digital mental health care doesn’t have to be a mirage. By moving beyond superficial solutions, we can create tools that genuinely transform lives. The question isn’t whether these tools have potential—it’s whether we’re ready to build them right.
