Join the 'Collaborative Conversation' - upcoming webinars for staff across the Integrated Care System

Across the Hampshire and Isle of Wight Integrated Care Partnership we have been exploring how we can use the home as a founding determinant of health and wellbeing.

By recognising that ‘health begins at home’ we have been successful in enabling people to live independent, flourishing lives. The first two videos in the series have proved popular and you can watch them here:

Watch back Session 1: Integrated Care - 'The Importance of Home to Health'

Watch back Session 2: The NHS Housing Hub

There are further 'Collaborative Conversations' sessions scheduled over the next few months. Sign up here.

Upcoming:

  • 12 December 2023 / Housing: A founding determinant of mental wellbeing
  • 24 January 2024 / Avoidable admissions (Hospitals)
  • 7 March 2024 / Minding Every Gap: Reducing re-offending through homelessness prevention

The sessions are open to all staff in health and social care, and partners like housing associations and other services as part of the wider system. Find out more and sign up here.

Marking World COPD Day this November

COPD (chronic obstructive pulmonary disease) is a group of lung conditions that make it difficult to breathe because your airways have been narrowed. Life with COPD can be difficult and everyday tasks can become a challenge. It's a progressive condition, which means there is no cure.?

That’s why every year in November we mark World COPD Day. This year’s focus was “Breathing is Life – Act Earlier". Our teams were out at locations across Hampshire and the Isle of Wight running fun competitions for people to pit themselves against staff. They promoted resources and information on COPD and COPD management, community and peer support groups, smoking cessation and help to get active.

Are you a healthcare professional who wants to learn more about supporting people living with breathlessness??

A free online course is now available that aims to help health professionals understand how chronic breathlessness can still be improved even when the underlying condition cannot. The training takes approximately 20 minutes to complete and is relevant for any health professional who looks after people with long-term conditions causing chronic breathlessness. This includes nurses, therapists, advanced practitioners, doctors and psychologists working in primary care, community care and secondary care.

Raising Voices in Research

Image from a previous Raising Voices in Research event.

Raising Voices in Research established a Research Collaborative (RC) of project partners, voluntary, community and social enterprise (VCSE) sector organisations, local government, and researchers. Since its inception, the RC has grown to 51 members, attracting new joiners who were not part of the original work and demonstrating its growing reach regionally.

With Raising Voices in Research being funded for phase 2, the Hampshire and Isle of Wight Integrated Care Board (ICB) Community Involvement team will also host two online sessions of the RC to bring ICB strategic pieces of work to the group. This will continue to embed research and community partners in ICB work and gain wider engagement and dissemination for these projects.

The first RC of phase two was held on 6 November 2023. The Integrated Care Partnership and other organisations came together to explain and showcase their work, and the fantastic opportunities available.

Nicky Judd and Jessica Johnson from Action Hampshire, the lead VCSE organisation and Project Co-ordinators of Raising Voices in Research, were sharing important updates about the next phase of the project at the event.

Fran White spoke about the Hampshire and Isle of Wight’s Integrated Care Partnership Update and priorities. Her presentation touched on the how the Integrated Care Partnership links in with the ICB and System.

Charlotte Brooks from ROVER, spoke about their work, which you can read all about on their website.

Claire Currie shared one of the Integrated Care Partnership's priorities, Social Connectedness. This programme is advocating an asset-based community development approach, which means recognising, connecting and mobilising the strengths, gifts, talents and resources of individuals and communities to build stronger and more sustainable communities.

Look out for future updates on this collaborative.
