The concept that errors should not always be labeled as "human error" stems from the understanding that errors often arise from the broader system and its processes, rather than solely from individual actions. Since human fallibility is inevitable, even in the most advanced organizations, it is more constructive to focus on the underlying factors contributing to errors. My proposal is to eliminate the term "human error" from analytical maturity frameworks and replace it with "human bias." This shift acknowledges that collective biases, systemic inefficiencies, and design flaws collectively create the conditions that lead to errors.
