If your organization is participating in a value-based care program, clinical staff and also support staff time between now and the end of year should be focused on the top priorities for Value-Based Care (VBC) Return on Investment (ROI):

Top Priority

Utilize support staff to:

  • Schedule patients for a clinic encounter (preferably a wellness encounter) for end of year (September-December).
  • If HCC Coding notifications at the patient level is not integrated into EHR, have the support staff print off patient face sheet identifying the HCC diagnoses not yet recaptured during the current year to bring to the clinical team's attention to review and re-code if applicable during the encounter.

  • If incomplete quality measure notifications are not integrated into the EHR, have the support staff print off patient face sheet identifying the incomplete or non-performance quality metrics for the patient to bring to the clinical team's attention during clinical encounter.

Quality Care Gaps

  • Obtain list of prescribed medications at patient level that are no longer being picked up by the patient to bring to the clinical staff attention for a discussion with the patient during the clinical encounter regarding the necessity of taking maintenance medications.
