WHAT - WHY - WHERE - HOW - WHO - WHEN - HOW
The problem can be real and apparent. At first, you need to state the problem specifically. If needed, consult each faction involved for information. Differentiate facts from opinions. Identify the outcomes of the problem.
Ask “Why” and find the cause. Then ask “Why” and find the cause behind the cause. Keep asking “Why”s until you find the root cause. You can use the Fishbone diagram or concept mapping for more complex problems.

When you have the root cause, identify the possible solutions. Pros and Cons, Weighted Rubric, SWOT analysis are used to find the best solution.
When you have the best solution, develop SMART goals that you can execute. Figure out which goal is the best.
Find who is assigned to execute the goals, who will supervise, who are responsible, who are affected etc.?
Set realistic dates and times to achieve the goal. If the goal is extremely complex, use the GANTT chart.
5W2H is an iterative model. You constantly need to monitor and measure the effectiveness of your process. First, ask if the problem is still there. If the symptoms/effects are not solved, then our solution is not effective. We must revisit our problem-solving process. Start measuring from the last step. Was the goal executed properly? Was the solution right? Are we still confident that the root cause of the problem is the cause that we have picked?
