Reading the article made me think about the central role that quality (and the QMS) has when assessing mission capabilities (of rolling stock): the reliability and safety analysis are vastly based on assuming that most of the failures of electromechanical and mechanical/pneumatic components are randomic (we leave electronics aside). When quality fails, and these failures become systematic or the common cause contribution heavily increases…..ALL evaluations (including safety) are just wet paper.
