How to convey bad news on project to team and seniors

The first step in delivering bad news or reporting mistakes is to?acknowledge the problem and take responsibility for it. Don't try to hide or blame others for what went wrong. Instead, be honest and transparent about the situation and how it affects the project objectives, timeline, budget and resources.
Be genuine, try to be authentic & compassionate, treat team / seniors with respect and dignity. Do not try to sugar coat the truth, be honest and transparent about what has happened and how you are going to over the problem and make it right.
Based on my real time experience,
1)???? Prefer one on one meeting, preferably face to face or video conference, find private location to discuss, try pick right time if feasible. Open with buffer statement.
2)???? Do not delay because news spreads fast.
3)???? Use right tone and be clear.
4)???? Discourage emotional response, maintain calmness.
5)???? Put yourself in other person’s shoes. Ensure news reaches to all stake holders on project.
6)???? Practice feedback in advance.
7)???? Propose, provide alternate course of action(s).
8)???? End politely, let it be constructive
