There are hazards you can readily identify in this video. Besides, conducting JHA, developing SOP and implementing it could be some of proactive solutions. The problem is, we do it everyday and get so used to of it that we tend to develop high risk-tolerance for the job resulting in risk taking behavior which we may not be aware of. One of the best solutions is to create or design conditions such that one has reduced probability of developing high risk tolerance, building unsafe behavior or conducting unsafely.
