The Healthy Aging Action Team, part of Waukesha County's Community Health Improvement Plan (CHIP), focuses on promoting fall prevention and fostering social connections for older adults. By collaborating with community partners, the team works to create resources and programs that empower seniors to stay active, safe, and engaged. These efforts aim to enhance the quality of life and independence of Waukesha County’s aging population.

Goals & Strategies

Fall Prevention Initiatives:

  • Promote evidence-based programs like Stepping On, Tai Chi for Balance and Ballroom Basics.
  • Partner with healthcare providers and various community organizations to educate older adults on home safety and fall prevention techniques.
  • Advocate for improved infrastructure, such as safer sidewalks and accessible public spaces.

Enhancing Social Connections:

  • Support community events that encourage interaction among adults.
  • Partner with local organizations to develop intergenerational programs like meal gatherings, hobby groups, and volunteer opportunities.
  • Explore digital tools and workshops to help older adults stay connected with family and friends.

CHIP ARPA Grant Recipients – 2024 Healthy Aging Connections

The following organizations received CHIP ARPA Grants in 2024 to support initiatives aligned with the goals of the Healthy Aging Action Team:

City of Waukesha Focus: Decrease falls and increase social connections & Expand Ballroom Basics to promote balance, coordination, and social engagement

Eras Senior Network, Inc. Focus: Enhance social connections for individuals over 60. Conduct outreach through telephone check-ins and foster intergenerational connections.

La Casa de Esperanza, Inc. Focus: Increase intergenerational social connectedness between middle school and seniors in Seniors and Scholars Program.

Advocate Aurora Health Focus: Prevent falls through education and awareness through the development of the Waukesha County Falls Prevention Coalition.

Universities of Wisconsin Focus: Decrease falls among older adults. Expand the StrongBodies program, emphasizing strength-building and balance.

These grant-funded efforts play a vital role in supporting the health and well-being of Waukesha County residents across all ages.
