Health Equity EHR Alerts
The healthcare industry has numerous patient data points collected from claims and clinical data, so the obvious technology agenda is to put these data points in actionable formats such as EHR alerts or notifications for easy consumption by healthcare staff and providers.
Health Equity Plans and other value-based care agendas are not difficult to achieve when the right data at the right time is delivered to healthcare providers and staff to act upon in their EHR workflow.
For example, REACH ACOs must put a Health Equity Plan in place that defines the data sources to be used and how they will identify and select patients for related health disparities services. Again, the obvious and easiest process is to integrate these processes into the EHR workflow.
If a patient's health disparity has already been coded as a Z code, then the historical claims data can prompt such disparity in the EHR workflow for the healthcare staff to act upon such as homelessness and providing shelter resources to the patient.
