Company fined £300,000 after explosion seriously injured two employees

A company which produces electricity from food waste by anaerobic digestion, has been fined more than £300,000 after an explosion caused two employees to suffer life changing injuries. On 20 September 2017, two employees of Bio Dynamic were using a grinder to cut and replace pipework at the top of an 11-metre high metal tank containing waste slurry. Sparks from the grinder ignited flammable gasses causing the tank to explode.

Companies fined a combined $48,000 after two people injured in oil and gas site explosion

Health and safety regulator WorkSafeBC has fined two Canadian companies a combined CA$48,000 (£27,000) after an explosion at an oil and gas site injured two people in June. According to a WorkSafeBC investigation, both companies failed to ensure the health and safety of all workers at the site when workers began welding above a tank which contained flammable chemicals.

Over 12 tonnes of hydrogen sulphide released in fatal incident, investigation says

The US Chemical Safety and Hazard Investigation Board (CSB) has released an update on the agency’s investigation of the fatal release of toxic hydrogen sulphide gas at the PEMEX Deer Park Refinery in Texas on 10 October which killed two workers and injured 13 others.

South Korean steel plant suffers second fire in two weeks

A facility operated by one of the world's largest steel manufacturers, Posco, in South Korea’s North Gyeongsang Province experienced a second fire in as many weeks on 24 November. No injuries were reported during the incident, however it comes after a fire broke out at the plant in in the city of Pohang on 10 November which left one worker with burn injuries.

Investigation update released into chemical fire that forced mass evacuation

The US Chemical Safety and Hazard Investigation Board (CSB) has released an investigation update into the chemical reaction and toxic gas release in September at the Bio-Lab, Inc. facility in Conyers, Georgia, that resulted in a massive fire and plume of toxic smoke that threatened the surrounding community and the metropolitan Atlanta area.
