Pemex refinery blaze injures four

A fire at a refinery operated by Mexican state oil company Pemex injured four people on 23 May. The incident happened at the company’s Lazaro Cardenas refinery in the city of Minatitlan, eastern Mexico.

Final report into 2021 fatal acid release published

The US Chemical Safety and Hazard Investigation Board (CSB) released its final report into the 27 July 2021 fatal acetic acid release at the LyondellBasell La Porte Complex in La Porte, Texas. The CSB’s report found that the inadvertent removal of pressure retaining components of a valve caused the release of 74 tonnes of an acetic acid mixture, killing two contract workers.

Refinery explosion injures two in US

An explosion and fire in a gasoline hydrotreater at a refinery in Oklahoma, US injured two workers on 23 May. The incident happened at CVR Energy’s Wynnewood site, 67 miles (107km) south of Oklahoma City.
