General Facility Safety Rules

?

1)??????? Immediately report all accidents, injuries, near miss incidents, illnesses or property damage to your supervisor.

?

2)??????? Report any unsafe practices or conditions believed to be unsafe or unhealthy to your supervisor.

?

3)??????? Maintain good housekeeping practices throughout the facility at all times.

?

4)??????? Clean up wet spots immediately, barricades must be set up in those areas that are wet, damp or still in the process of drying.

?

5)??????? Clean up all oil spots/spills immediately. Contact your local emergency coordinator (EmC) for the proper disposal policy.

?

6)??????? Clean up all hazardous material spills by immediately contacting the local emergency coordinator (EmC) or the local incident spill response contact. Only trained individuals are authorized to clean up incidental chemical spills. Hazardous material spills that constitute an emergency contracted hazardous material cleanup firm.

?

7)??????? Obey all traffic rules and warning signs.

?

8)??????? Use protective equipment when performing the following or similar tasks.

?

a)??????? Protective aprons, goggles, face shields, gloves, etc.

?

9)??????? Wear safety shoes when working in environments where required or when job tasks necessitates foot protection.

?

10)???? Food, Beverages and smoking are permitted in designated areas only. Use of smokeless tobacco is prohibited in work areas.

?

11)???? All unattended file cabinets must be kept closed.

?

12)???? All cords should be secured to the sides or underneath the desks to prevent tripping.

?

13)???? Desks should always be located over receptacles to prevent tripping.

?

14)???? Use only 3 prong grounded power tools, or double insulated case, U.L. approved, reverse polarity protected tools. All electrical cords must be free from cuts, splices and exposed wires.

?

15)???? Extension cords (excluding power strips) may be used in temporary, single-use situations only.

?

16)???? All aisles and walkways must be kept clear of tripping hazards such as electrical cords, hoses, pipes, boards, loose carpeting, etc.

?

17)???? Never repair, service, test, or work on machinery unless authorized to do so by your supervisor.

?

18)???? No safety device is to be blocked, bypassed or otherwise altered.

?

19)???? Horseplay or running in and around all areas of the facility is prohibited and will not be tolerated.

?

20)???? Jewelry (i.e., rings, loose bracelets, long necklaces worn outside clothing), should not be worn when operating machinery with moving parts or when working in an environment with: production machinery, conveyors, platforms, hand held or stationary tools and ladders. Additionally, loose clothing should not be worn or should be tucked in. The purpose is to prevent jewelry or clothing from catching on equipment or controls which could possibly result in serious injury.

?

21)???? A fall barrier (chain, bar, fence, etc.), toe board and signs must be in place wherever there is a drop of more than three feet, (i.e. mezzanines, etc.).

?

22)???? Individual heaters (i.e. under desk or free-standing units), may only be used if its operation is in compliance with local fire codes.

?

23)???? Only authorized personnel are allowed in the distribution center. Non-authorized personnel must remain in designated areas.

?

24)???? The use of headphones or other similar devices and headphones while working in a prohibited.

?

25)???? Wearing of contact lenses is prohibited for those employees working with or around chemicals or the presence of chemical vapors.

?

Safety Rules-Material Handling and Equipment:

?

1)??????? Do not climb or stand on products, pallets, or pallet racking.

?

2)??????? Do not exceed weight capacity of dock boards. Dock plates and load levels must be inspected for cracks and stability before using.

?

3)??????? Prior to loading into trailers, check the trailer floor for integrity to ensure there are no holes or areas that would be compromised with the added weight of a, load or forklift traffic in the trailer. Should floor integrity be compromised, seek direction from supervisor prior to proceeding. Load and material handling equipment should never be placed in trailers with holes in the floor.

?

4)??????? Exercise extreme caution when operating forklift and material handling equipment in populated areas of the any environment.

?

5)??????? Electric pallet jacks must be secured inside the trailer when in transport.

?

6)??????? Keep feet clear from pallets in motion.

?

7)??????? Never stand or walk under elevated forks or loads, or across forks.

?

8)??????? Check pallet loads for looseness and stability.

?

9)??????? Before driving a forklift, always test brakes, steering and horn; check fire extinguisher and other controls for proper operation. Any forklift which needs repair must be taken out of operation and report to your supervisor.

?

10)???? Do not carry loads higher than 3 inches off the floor, or three inches above the outrigger tires when operating a narrow aisle. Tilt the mast backwards to support the load, and do not drag loads across the floor.

?

11)???? When operating a narrow aisle lift, ensure reach is retracted prior to transporting a load. Do not allow the pallet to drag the outrigger tires.

?

12)???? Employees must always secure safety belts/tethers/harnesses when beginning work on order pickers.

?

13)???? Always look in the same direction of the path of travel and travel with forks behind you when a load is being carried.

?

14)???? Observe overhead clearance (i.e. Sprinkler heads, heaters, etc.).

?

15)???? Always maintain at least a three, vehicle length distance between forklifts.

?

16)???? Do not touch, lean on or reach through a forklift mast. All body parts, arms and legs, must remain inside the vehicle at all times when in motion.

?

17)???? When parking a forklift, park in designated areas, (not aisles), lower forks to the ground, take the forklift out of gear, apply the hand brake, (if available), and turn the forklift off before dismounting.

?

18)???? Before driving a forklift, make sure surrounding area is clear and ensure the tether is hooked to the employee’s safety belt.

?

19)???? Never carry passengers on a forklift except when using a safety cage to lift personnel.

?

20)???? Never carry more than two full pallets at a time. These would mean pallets, which are no more than 3’ high each.

?

21)???? Slow down and sound horn at all corners, passage ways and all main intersections. Slow down, yield to other material handling and/or pedestrian traffic, and sound horn before passing through the firewall or blind intersections.

?

22)???? Squarely place pallets in racks so they will not fall.

?

23)???? Come to a complete stop before dismounting forklift and do not use seat brake to stop a forklift in motion.

?

24)???? Exercise extreme caution when getting on or off a forklift to prevent hitting any part of your body.

?

25)???? Forklifts and any other equipment that is used with chemicals must be rated EE.

?

Dock Safety Policy

?

This policy covers both the loading/un-loading of any and all vehicles either making deliveries to or loading to make deliveries of company products.

?

This Policy also, covers the safety of any/all employee’, vendors, truck drivers, to include anyone working on, waiting for, or just in passing, to include all vendor, contractors, temporary employees, visitors, etc.

?

This policy is being implemented is ensure the safety of all employees, as well as, all delivery/pick-up drivers safety.

?

  1. No one is ever allowed to climb in or jump off any of the docks. Only entering/exiting by stairway/ramp exit.

?

  1. Never stand to close to the edge of the dock, i.e. half on, half off.

?

  1. Never throw any trash from the dock doors.

?

  1. Never stand outside of the dock on the ground pending for any reason.

?

  1. When cleaning the outside area where a dock door is, ensure you are at all time observing vehicle movement.

?

  1. Never park any vehicle at a dock door and leave it without approval from Warehouse, who will know scheduling for deliveries/pick-ups.

?

  1. At all times when a Dock is not in use, there should be a gate closed and locked across the dock area, i.e. upon the dock with a Yellow Caution Sign pointing into the warehouse, as a warning for those walking by of a drop off. The barrier must also have pointing outward a safety sign advising safety.

?

  1. When a delivery/pick-up driver arrives at the dock, the driver must turn off his/her engine, ensuring the parking brake is on.

?

  1. The driver is to report to the Shipping/Receiving office signing in on the sign in log.

?

  1. The drive must turn in his/her keys, allowing for the employee who will be performing the unloading/loading of their vehicle to place the keys in the driver lock box, until the loading/unloading and final paperwork has been signed.

?

  1. The employee to perform the loading/unloading of the vehicle is to ensure the drive chocks both his/her back tires with the Chocks provided, ?THIS IS AN OSHA REQUIREMENT.

?

  1. Only after the employee/driver ensures his/her vehicle tires have been chocked can the loading/unloading begin.

?

  1. Upon completion of the loading/unloading process to include completion of all final paperwork, and only at the time when the driver of the vehicle is ready to leave the dock, will the keys be returned to the driver.

?

  1. The OSHA Required Safety Chocks be removed, and the driver leave.

?

  1. In the Ramp Door area, there is to be a metal scissor gate Barrier placed across the dock door area, and must be locked, whenever the ramp is not in use.

?

  1. When unloading/Loading company vehicles the same policy is to be upheld for company Dock Safety.

?

  1. When unloading/loading any vehicle in the parking lot, the driver must follow all the above standards as if his/her vehicle were at a dock door.

?

?

Violation of any part/parts of this policy by and driver, vendor, 3rd party, could result in that person/vendor not being allowed to return.

?

Violation by Company employee’ will result in disciplinary up to and including termination.?

?

Powered Industrial Lift Truck Training And Company Certification Policy/Processes:

?

  1. Each FTE, hire who will be performing Company Powered Lift Trucks, will first have to complete a Certification training course.

?

  1. The Certification Training must be conducted by an approved trainer.

?

?

The follow is a list of operator Training guidelines: (OSHA Required)

?

  • The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this training.
  • The trainer prior to permitting an employee to operate a powered industrial truck, shall ensure that each operator has successfully completed the training required by the Company.
  • Trainees may operate a powered industrial truck only: Under the direct supervision of persons who have the knowledge, training, and experience to train operators and evaluate their competence. Where such operation does not endanger the trainee or other employees. Training shall consist of both written and practical training and evaluation of the operator’s performance in the workplace.

?

  1. Training Program Contest: Truck Related Topics: Operating instructions, warnings, and precautions for the type of truck the operator will be authorized to operate; Differences between the truck and the automobile; Truck controls and instrumentation: where they are located, what they do, and how they work. Engine or motor operations; Visibility Fork and any attachments, operations and limitations; Vehicle capacity; Any vehicle inspection and maintenance that the operator will be required to perform; refueling and/or charging and recharging of batteries; Workplace related topics; Surface conditions where the vehicle will be operated; Composition of loads to be carried and load stability; Load manipulation, stacking, and unstacking; Pedestrian traffic in areas where the vehicle will be operated; Narrow Aisles and other restricted places; Hazardous locations where the vehicle will operate; Ramps and other sloped surfaces that could affect the vehicle’s stability; closed environments and other areas of insufficient ventilation or poor vehicle maintenance could cause buildup of carbon monoxide or diesel exhaust;

?

  • Refresher training and evaluation: The operator has been observed to operate the vehicle in an unsafe manner; The operator has been involved in an accident or near-miss incident; The operator has received an evaluation that reveals that the operator is not operating the truck safely; A condition in the workplace changes in a manner that could affect safe operation of the truck; An evaluation of each powered industrial truck operator’s performance shall be conducted at least once every three years.

?

  • Certification:

The employee shall certify that each operator has been trained and evaluated as required by the above program. The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person (s) performing the training or evaluation.

Types Of Lift Truck Equipment used at facility.

?

?

Training Knowledge and work sheet for Training employees

?

  1. An average sit-down, counterbalanced forklift weights approximately 9,000 lbs.

?

  1. The Average Capacity for this truck is 5,000 lbs.

?

  1. Many forklifts steer with the rear wheels, which causes the rear end to swing out.

?

  1. Forklifts can be battery powered, or fueled by propane, gasoline or diesel.

?

  1. Forklifts have tires, some have tread, and some do not.

?

  1. Forklifts can have mirrors, lights and alarms, and must have a horn.

?

  1. Use of lights on forklift when lighting is poor, when traveling, or anytime usage would increase safety of the operator and others.?

?

  1. Ensure all emissions from forklift are within safety standards, i.e. there should be no smell of any fuel of any type, when getting ready to us a fork truck or while usage is going on.

?

  1. Lifts are never to be driven up to another person while standing in front of anything, i.e. bench or fixed object.

?

  1. No person should ever be allowed to pass under the elevated portion of any truck whether loaded or unloaded.

?

  1. Unauthorized personnel shall not be permitted to ride on powered industrial trucks. A safe place will be provided when training is taking place away from any work station or others.

?

  1. No one should ever have their arm’s or leg’s, etc. placed between the mask/lifting area, or outside the truck while in operation.

?

  1. When any truck is not attended, the load must be lowered all the way down, engine off, park brake on.

?

?

  1. OSHA standards are that an unattended truck is when its driver is off the truck more than 25’ or more away from the truck, which may remain in his/her view, or whenever an operator leaves the truck and it is no longer in his/her view. Policy is whenever an operator leaves the truck for any reason, the truck should be turned off, forks lowered, park brake on!!

?

  1. A safe distance shall be maintained from the edge of ramps or platforms while on any elevated dock, or platform or freight car. Trucks shall not be used for opening or closing freight doors.

?

  1. Brakes shall be set, and clocked whenever loading or unloading any type of material from any type of moving trucks, trailers, and or railcars.

?

  1. An overhead guard shall be in place to protect against falling objects.

?

  1. Only approved industrial trucks shall be used in Hazardous areas.

?

  1. If at any time a powered industrial truck shall be in need of repair, it is to be locked out and tagged out, and not placed back into service until it has been repaired.

?

Safety Continued; Safety in the Facility;

?

  1. Whenever lifting another employee for any reason, there must be a platform with side rails used.
  2. The employee being lifted should have on a harness can be hooked to both the side rail of the platform and around the employee, to ensure in case of fall, the harness will stop the employee from falling all the way to the ground.
  3. The employee being lifted should have PPE to ensure while being lifted, any falling material, or other forms of objects will not harm the employee.

?

Safety while operating a lift truck throughout the Facility;

?

  1. Fire aisles, access to any stairway, and fire equipment must always be kept clear at all time, i.e. when a lift truck is not in the process of performing a job assignment and not be operated, it should never be left in an area to block these types of areas.

?

  1. All traffic regulations shall be observed, including authorized plant speed limits.

?

  1. A safe distance shall be kept between industrial lifting equipment, i.e. at least three, (3), truck lengths.

?

  1. Never pass another truck in blind spots, i.e. corners, walkways, entering or exiting a building, etc.

?

  1. The drive must slow down and sound horn at crossways, or any other location where vision is obstructed.

?

  1. The driver must at all time look in the direction of travel ensuring a clear view at all times.

?

  1. At all time when driving the truck shall be driven in a manner that at any given time stopping in a safe manner can be performed.

?

  1. Stunt driving and horse play will not be permitted.

?

  1. The drive will be sure to slow down for wet or slippery floors.

?

  1. Dock boards/plates shall be driven over slowly, and be properly secured before driven over.

?

  1. Running over loose objects on the roadway surface shall be avoided.

?

  1. Turns should be taken slowly at all times.

?

  1. Only stable loads and safe loads should be handled, when exercised off-center loads which cannot be centered, there should be at least two other employees walking with the load to ensure it is safe throughout the movement of the load. (Employee’s walking with an un-centered load, should ensure their safety at all times, walking with the load, but ready to easily remove themselves from danger if it were to arise).

?

  1. Only loads within the lifting truck’s capacity are to be moved, never try to move a load over the capacity of the lift truck.

?

  1. The back of the forks shall be reached as close as it can be to the load, with the load tilted backward to ensure the load is stabilized.

?

  1. At all time when tilting frontward or backward, care should be taken to ensure the load doesn’t move and cause it to fall over.

?

Fueling Safety

?

  1. Never fill any lifts or industrial lifting equipment while the engine is on.

?

  1. If any fuel is spilled during filling of the tanks it should be cleaned up at once, before the engine is started.

?

  1. No truck should ever be operated with a leak in a fuel source.

?

  1. No flames of any type should be used when checking the battery or battery area on any lift truck.

?

  1. Smoking is not allowed when operating any type of industrial lifting truck.

?

  1. No repairs shall be made in any other area of the facility except in the maintenance area of the facility.

?

  1. Trucks in need of repairs must have the batteries disconnected prior to any repair.

?

Inspections of Industrial Lifting Equipment

?

  1. There should be no alteration of any type performed on any industrial lifting equipment without formal written release from the manufacturer.

?

  1. A daily check of all industrial lifting equipment must be performed, before each unit is released for service each day. This check list should be performed daily by an approved knowledgeable person from the maintenance department.

?

  1. Records of such inspections shall be kept by the, (Safety Manager), for no less than (7) seven years.

?

  1. All Industrial equipment should be kept clean and free of dust. Lint, excess oil, and grease at all times, for trucks with these types of material have a much higher risk of fire in heated areas/summer heat!

?

Trailer Safety

?

  1. Before entering ensure the flooring is all in good order.

?

  1. Ensure that the trailer is secured in place by either chocks or the ICC bar lock, and the brakes on the tractor are set.

?

  1. If the trailer is to be dropped during the loading or unloading a jack should be used to support the weight in the front of the trailer.

?

  1. Provide adequate lighting by either the use of the lift trucks lights, or light directed into the trailer.
  2. Ensure GOOD COMMUNICATION between the tractor / trailer driver and the fork truck operator is extremely important.

?

  1. If there are other employees working inside the trailer, ensure they have a means of egress.

?

  1. Watch for pedestrians when backing out of the trailer.

?

All Training and evaluation must be conducted by persons with the necessary knowledge, training and experience to train powered industrial truck operators and evaluate their competence, this can be someone with a possession of a recognized degree, certificate, or professional standing, or who by extensive knowledge, training, and experience has demonstrated the ability to train and evaluate powered industrial truck operators.

Driver’s Company Vehicle Inspection Reports

?

  • Each day at the start of each driver’s shift, it is required that they perform a daily checklist of the Vehicle they will be driving that day/shift.

?

  • The form after being filled out will be turned in to the supervisor, who will look over the forms and pass them on as needed.

?

  • If during the time the driver if filling out the form, there is something found which needs attention right away, before the vehicle can be used, the driver at once should go to the maintenance department and advise of the needed. The Maintenance employee should get to this ASAP, in order to not hold up the driver’s schedule for that shift.

?

  • If the trouble is to big to be fixed at once, and will delay the driver’s schedule, the driver and the maintenance department employee should both ensure that the driver’s manager is advised at once, in order for the driver’s schedule to be handled in whatever manner the manager decides.

?

  • This Company Vehicle Inspection Report, in no way takes away the driver’s responsibility to fill out the DOT driver log, as needed.

?

?

Daily Check List to be filled out by the Company Maintenance department on each Industrial Lifting truck/equipment each day.

  • The Company Maintenance Department is to perform DAILY, a check list of all industrial lifting equipment.

?

  • At the start of each day, there is to be assigned one company maintenance employee to fill out and check each piece of company lifting equipment. The Maintenance employee is to determine, rather the equipment is both working right and safe for approved company employees to use throughout their shift.

?

  • If the lifting equipment is found to be un-safe or not in good working order, the maintenance employee should at once, LOCKOUT & TAGOUT, that equipment, not to be used until the equipment is both in good working order and safe to be used. Only the Maintenance department will be able to release the LOCKOUT TAGOUT, tag and unlock the material to be used again.
  • The Maintenance department will be in total control of all LOCKOUT & TAGOUT Keys at all times, i.e., see LOCKOUT & TAGOUT, PROCESS, POLICY and COMPANY RULES!

?

  • At least weekly the maintenance department will turn into the assigned Safety Professional all the check lists for that week; it will be the maintenance department head who will be responsible for ensuring these checks are performed daily.

?

  • The assigned Safety Professional will keep an ongoing list of repairs and costs to each industrial lifting truck, ensuring upper management is aware of these ongoing costs.
