From Registration to Recovery: How Stre@mline EMR is Improving the Patient Experience

The patient journey in the Stre@mline EMR typically begins with the registration of the patient, where their personal and demographic information is collected and entered into the system. This information is then used to create a unique patient record that can be accessed by authorized medical personnel.

Once the patient is registered, a new episode is created for them where they will typically begin their journey once their demographics are logged into the system. The patient is then Triaged by a nurse/ medical personnel where observations, symptoms, and patient vitals are recorded and then the patient is graded and prioritized based on illness/injury, severity, prognosis, and resource availability. Here they are also assigned a clinic.

The patient can then proceed to consultation with a Doctor at the respective clinic where they have been assigned. During this consultation, the medical personnel will use Stre@mline EMR to document the patient's symptoms, medical history, and any other relevant information. Here Stre@mline provides timely prompts for common diagnoses and also reference material for the Doctor. This recorded information is then used to create a treatment plan, which is also stored in the patient's episode.

As the patient receives care, any additional information such as investigations, radiology images, and other diagnostic test results, will be added to their electronic record. This allows for a more complete and accurate view of the patient's health status and treatment history all within Stre@mline.

The Patient can then go on to clear any outstanding medical bills at the cashier after medication depending on their category, the amount of care given and other parameters set for that health facility under the patient’s respective finance.

As an EMR, Stre@mline has been a powerful tool for improving the state of providing accessible healthcare. Stre@mline has increased the speed and accuracy of diagnoses and improved the overall quality of care. Additionally, Stre@mlilne has helped to reduce the burden on healthcare providers by automating many of the administrative tasks that are associated with paper-based medical records.

Stre@mline has improved the coordination of care within the health facility, by allowing multiple medical personnel to access and update the patient's electronic record in real-time depending on their authorization. This has especially been important in tracking patient history and timely data-driven reporting which helps the medical personnel make decisions even at critical moments when providing care for patients and streamlining the patient journey.
