From Guilt to Guidance: Supporting Your Loved One from Hospital to Rehab

Transitioning a loved one to skilled nursing or a subacute rehab center doesn't mean stepping away from their care. It means taking on a different role in their recovery journey. Here's how you can stay involved.

Engage with the Care Team: Nurses, therapists and social workers can help ensure your loved one's needs are met. Open, proactive communication fosters trust, and strengthens your role as a valued care partner.

Make Regular Visits a Priority: Whether in person or through video calls, maintaining consistent connection is key to their emotional well-being. If you were once a constant presence, strive to maintain that same level of engagement to provide comfort and continuity.

Be Their Voice When It Matters: If concerns arise, speak up. You are still their strongest advocate in making sure they receive the best care possible. You know their needs and preferences better than anyone, and you may be surprised at how receptive caregivers are to your insights.

Help Them Stay Active and Connected: Help your loved one get involved in social groups and other events to keep them engaged. Studies show that staying socially active improves physical health, cognitive function, and overall well-being.

By staying actively involved in these ways, you'll reinforce that your loved one is cared for and supported without carrying the full weight of caregiving alone. You'll give yourself the chance to reconnect, not just as a caregiver, but as a son, daughter, or family member first. A safe recovery and return home isn't just possible, it's happening every day.

This is a great piece of advice. I would add to start a journal and write down all your interactions with staff and be sure to show appreciation for when staff gets it right!!! Volunteer to help at a social event and ask your loved one to help you!!
