I have spoken at more conferences than I can count over the last 17 years on behalf of MostlyMedicaid.
I have addressed health plan executives, solution provider engineers, actuaries, and state officials. I have talked about fraud, social determinants of health, Medicaid financings, quality measures, and recently a lot about the PHE.
For the last two years, I have covered a range of Medicaid topics every Friday in my 5BF video segment. And for the 15 years before that, ?I wrote Clay’s Weekly Medicaid Roundup every Friday.
I have done countless interviews with Medicaid Directors, Governors, Federal HHS IT officials and every manner of stakeholder in this space over the years. There are thousands of hours of content in our catalog and you can learn a lot just by clicking a link in our weekly newsletter.
As wild as it sounds, about 10,000 people check out something we put out each and every year. About 2,500 or so of those are state, local or federal officials. Another 3,000 or so are Medicaid health plan executives and staff. Another 2,000 or so are solution provider staff. And the remaining 2,500 are a mix of direct care providers, health system execs, other consultants and who knows what else. We attract a motely crew, and would have it no other way.
I have been honored that all these people care what I have to say all these years. (Or maybe some of them are just wondering what I will say next- I am known for being a little different in how I speak about things).
But on Feb 6th, 2023 at 8AM Eastern time in Orlando, Florida- ?I will be giving the most important talk of my life so far.

For now I will give you a few nuggets to noodle in your noggin as you think about whether you want to come say hello in Orlando in a few short weeks.
But before that, let me tell you a little bit about the conference.
About the Medicaid Innovations Conference
First and foremost I must tell you it is the best conference in the space. I say that as no slight to the other dozens of conferences we have partnered with over the years. But there is one that is the best and it is the Medicaid Innovations event.
I say this with confidence because I have been to it nearly every year, and chaired it 10 years (maybe more I can’t remember). I don’t put my name to anything that much that I don’t believe in completely. Pam, Aron and team have put in the work over the years to make this event a powerful gathering of all slices of our space.
If you go, you will see health plan folks, state folks, techies, and a range of others. Its just the right size where you can have meaningful conversations, make connections and learn. If you are on the conference rat race like so many of us, you know this is a rare trifecta.

Why am I so focused on Medicaid Managed Care right now? ?
We have always had a large chunk of our audience and client network in the Medicaid managed care space. But the past few years have made it clear to me that managed care is absolutely the most viable path forward for the impact we are all trying to make.
Hear me out. (I see you advocates in the back getting antsy).
I know all the warts of managed care. I know them better than most.
Not only have I written about them for decades (including calling out clients who are health plans publicly when they screw up), but I have also been in the trenches helping plans get things out of the ditch.
I count many of the MCO CEOs and other C-suite members as dear friends and have learned so much from them about their struggles. But I have also learned about their resolve. And their commitment to Medicaid. And their vision on how to improve it.
Yes, Medicaid managed care can be a hot mess. And it probably is more a hot mess than its not.
But its our hot mess, and its way more a viable model to change things for members than fee for service (also known as un-managed care).
Disagree? ?Come see me in Orlando and let’s debate it over a drink or a meal.
States agree with me on this. So does CMS. Just look at the gigantic migration of members into managed care over the last 2-3 decades. Yes, a whole lot of this is because Big Insurance knows how to get cash shifted to its model. Duh. This is the way of the world my friends and to sit in the corner wishing it was different is fine for brand new MPHs with no experience in the actual Medicaid industry. But for the rest of us, it is high time to accept that this is our path. If we claim to have a desire to impact this space- we must play the hand we are dealt. And that hand is Medicaid managed care.
Managed care simply has levers and resources that government agencies never will have directly. And MCOs can be incented to help members. And members like it (mostly), and at least we have things like CAHPS to measure how much they do or don’t like managed care. We don’t really even have a clear line of sight on members’ opinions of fee for service.
