First Postmortem
So what is an autopsy? An autopsy is a written record of an incident. Among other things, it documents the impact of the incident, what caused it, the actions taken to mitigate or solve it, and how to prevent it from happening again.
We need to accept failure for autopsies to work. This includes having irreproachable autopsies and not pointing the finger when looking for fundamental causes. Human error is never a root cause.
When to write an autopsy? A good rule of thumb is to write one if an incident has an immediate impact on users (downtime or loss of data) or if an interested party requests it.
