The knee must maintain six degrees of freedom for the screw home mechanism to function in a healthy knee. When a patient has osteoarthritis of the knee, these motions are altered, therefore increasing pain and progression of arthritis. The arthritic knee also is extended more at initial contact than an asymptomatic knee, thus rapidly increasing the ground reaction forces and the peak knee adduction moment, which is also associated with the progression of osteoarthritis.
I have been an orthotist for over 40 years, and I have a patent on a bracing technique which I call the Rotation Realignment System (RRS). I have successfully used this method in controlling the triplanar movement of the lateral thrust gait and greatly reducing the peak knee adduction moment. I feel this technique will restore the natural movement of the knee, also known as the screw home mechanism. I actually wear bilateral KAFOs myself due to a spinal cord injury and I have had great results on my patients, as well as myself, in eliminating pain and realigning the muscles of the entire leg with braces designed from my system.
