The FIVE Finger Model, and it's approach to risk reduction and prevention of dementia is the result of a multi-country study led by professor Miia Kivipelto at the Division of Clinical Geriatrics, Center for Alzheimer Research, Karolinska Institute in Stockholm. October 14th to 25th of 2024, we welcome Ministry of Health Delegation from Malaysia to Sweden's beautiful capital for an in-depth exploration of Swedish dementia care, facilitated by us at Swedish HealthCare Academy. About this program
This program will focus on exploring various dementia care models and intervention as well preventive strategies in different healthcare settings across in Sweden. The objective is to give the participants an introduction to the Swedish dementia care and to showcase new developments and innovations with focus on care for the elderly.
The aim is to learn more about:
– Sweden’s innovative approaches in dementia care, particularly in the areas of patient-centered care.
– Preventive strategies at primary health care level, cognitive rehabilitation, and community-based dementia programs.
Participants will engage in site-visits to Swedish elderly care facilities as well as visiting primary care units that work with dementia care. In addition, meetings will take place with experts and professionals conducting research and development within dementia care.?
Organizational profiles
Sabbatsberg geriatric clinic
Sabbatsberg geriatric clinic specializes in geriatric care and rehabilitation, offering examinations, treatments, and care for individuals over the age of 65 in both outpatient and inpatient settings.The care departments focus on heart failure/COPD, orthopedic conditions, stroke, and cognitive diseases. One of the wards is specifically designed for patients carrying MRSA, a bacterium resistant to several types of antibiotics. All departments have a broad emergency geriatric mission and are equipped to provide care for patients whose chronic illnesses have worsened or who are newly ill with acute problems.
Elderly care administration, City of Stockholm
Elderly care services in the City of Stockholm are for people who have reached the age of 65 and require help and support in their daily lives, or who need to move into a different kind of housing. Examples of elderly care services include personal security alarms, home care services, daytime activities, and nursing and care homes. The City of Stockholm charges a fee for elderly care services.
Ersta Hospital
The hospital is owned by Ersta diakoni and its operations began as early as 1864. The new part of the hospital, which opened in 2023, covers approximately 23,000 square meters. The hospital includes a pharmacy and café on the ground floor, an underground garage, a logistics center in the basement, patient wards, a dining hall at the top, and several rooftop terraces.
Specialties offered at the hospital include: Palliative Care, Psychiatry and Addiction Treatment, Geriatric Medicine, Rehabilitation Services and Home Care Services etc..
Ersta diakoni is a non-profit organization that operates healthcare, social work, education, and research without aiming for profit.
Danderyd Hospital?
Danderyd Hospital is one of Sweden’s largest emergency hospitals and provides university healthcare distributed across twelve operational areas. Annually, the hospital receives approximately 90,000 emergency patients, 429,000 outpatient visits and 50,000 inpatient visits, and performs 6,300 deliveries. In collaboration with the Karolinska Institutet (KIDS), education and research is conducted around the most common public diseases. Annually, roughly 2,000 students from roughly 100 different institutions of higher learning are welcomed in about 30 different educational programs. The hospital was founded in 1922 as Stockholm County’s central hospital in M?rby and was renamed in 1964 to Danderyd Hospital. Danderyds Sjukhus AB is a wholly owned company within Region Stockholm and has around 4,500 employees.
“Train the brain’’ program, Liding? municipality
Liding? city is the first municipality in the country to implement an evidence-based lifestyle program to counteract memory problems in the elderly. The aim is to prevent dementia in people in the risk group. The city of Liding? invests in dementia prevention and, in collaboration with researchers at Karolinska Institutet, has developed a lifestyle program for seniors. After a successful pilot project, the program will become a permanent part of the city’s operations starting in the fall of 2022.
The lifestyle program has been named Tr?na Hj?rna (”Train the brain’’ in English), is based on the FINGER model and consists of: physical and social activity, diet, cognitive training and management of vascular risk factors.

L?jtnantsg?rden is a long-term care facility owned and operated by the Immanuel Church in Stockholm as part of their social ministry. The facility consists of 5 accommodation units, two larger units for somatic diseases with a total of 28 apartments, and three smaller units for dementia, with a total of 26 apartments. The environment and activities in the dementia unit are specifically adapted for people with dementia. The apartments are 27–34 square meters, and each is equipped with a French balcony. The staff caters to the needs of the residents individually, and strive for residents to have a dignified life and the best possible well-being every day.?
Through the Immanuel church, residents and staff have access to a deacon and pastor for conversation and support. L?jtnantsg?rden is an idea-based organization and operates as a non-profit.
Silviahemmet?
Silviahemmet is a non-profit foundation devoted to improving the quality of life for persons affected by dementia and their families. The Foundation was inaugurated by H. M. Queen Silvia in 1996.
The Silviahemmet care philosophy aims at highest quality of life for the person affected by dementia and the family. Silviahemmet runs a daycare center at Drottningholm, for elderly as well as young persons affected by dementia. The foundation – in cooperation with academic institutions – educates assistant nurses, nurses and physicians in dementia care. Silviahemmet also provides non-academic education programs for professionals in the health care and social care sector.
Silviahemmet has also co-founded the Swedish Dementia Center, a national competence center for dementia.
Swedish Dementia Centre
The Swedish Dementia Centre is the national centre for excellence in the field of dementia. The centre collects, structures and disseminates knowledge about dementia. One of our principal tasks is to strengthen the links between research and practice and to communicate evidence based knowledge in a practical setting to health and social care givers. In collaboration with authorities and organisations, we work to inspire and promote the development of a more dementia-friendly society.
Dagsk?r dementia daycare center
Dagsk?r operates a dementia daycare in the center of Gustavsberg. They offer morning coffee and lunch from Gustavsg?rden’s restaurant. The dementia day care at Dagsk?r involves personalized assistance. Once a decision on assistance is made, the staff conducts a home visit to understand the individual’s needs.?
The day usually starts with morning coffee and conversations, followed by a group walk with varying goals and distances. Throughout the day, a range of activities take place, including group and individual activities such as sit-ups, balloon gymnastics, bowling, boule, singing, music, tactile hand massage, card games, painting, and more.
