End-of-Life Forms: What's Needed?
There are two essential documents for ensuring your aging loved one's end-of-life wishes are met and respected by EMTs and hospitals - a POLST and a DNR.
So what is a POLST form?
A Physician Orders for Life-Sustaining Treatment (POLST) is a one-page form that specifies what the individual wants or doesn't want.
Hospitals and EMTs must comply with the instructions of the POLST because it is a written doctor's order. Typically, this form is best for the terminally ill or the severely frail who've made their end-of-life wishes clear.
What about a DNR?
The Do Not Resuscitate form is a signed medical order written by a doctor. Health care providers and emergency medical personnel must follow stated directions not to perform CPR on the individual if they stop breathing or their heart fails.
The DNR only gives instruction for CPR and does not affect any other treatment. If your aging loved one would like more direction for hospitals and emergency personnel pertaining to end-of-life treatments, they should choose a POLST.
