Wow – a lot has happened this year and we’re looking at more changes starting January 1st. Most of it is GREAT NEWS too if you are hoping to either increase the clinic income or reduce the workload on your doctor or practitioner.

If you are not taking advantage of the $240 that Medicare is paying when you do a cognitive impairment assessment this year, you really show – especially since it’s going up to $270 next year as an average. Ok – in some places it will be down to $255 and others it will be $360 – depending on where you are in the country. This does take about half an hour of the time for the MD, DO, NP or PA – but even so – that is GREAT as it puts their time at a value of $540 per hour. Think about it. ERISA
Some people reading this are STILL letting insurance carriers recoup on their claim – simply because they are not aware that it’s against the law for them to do so. WAKE UP. If I sent you an invoice saying you owe me $14,000, there is no way you’d be dumb enough to just believe it and send me a check – without checking it out to see if you truly owed me. Yet – you and your hospital and clinic do that all of the time. Some are also believing the lie that they have a 90 day or 120 day claims filing deadline because the “contract” they have with the carrier says that. That contract portion is negated because of the ERISA law – but don’t hold your breath waiting on the carrier to tell you that you do not have to abide by it. The patient’s policy is ALL that dictates the filing deadline and most of them are a year and some are two years. TAX CODE 179 DEADLINE IN 2 WEEKS’ TIME
If you buy a device and put it into service this calendar year (2018), you can deduct the full amount that you financed on that equipment, regardless if you don’t start making payments till next year. That reduces how much you will be paying in tax next spring. At this time of year, we sell a lot of ABIs and ANS devices. It takes 2 ABIs a month for you to break even on the equipment and most of my primary care doctors are doing 1 to 2 a day – so they are making $4K a month net profit. It only takes 4 ANS tests a month to break even and that usually brings in another $8K a month net profit. Don’t wait till January if you are looking for ways to reduce your taxes and increase your clinic income – while helping your patients. It doesn’t make sense that you’re sending patients to other doctors so the other doctors get to make the $48K a year or $96K a year profit. Call me today at 903 871-1172 and let’s talk.
MEDI-MEDI PATIENTS WITH DIABETES & HYPERTENSION
Many patients do not routinely check their BP or blood sugar at home until it is the week that they’re about to come back to your office for a follow-up. Medicare knows this and so does Medicaid. We have a program whereby you can have the HHA set up the patient on remote monitoring and you get the reports on the patient. In Texas, the Medicaid program pays the doctor about $228 a month for it. Medicare is going to start paying for remote physiologic monitoring starting January. PRIMARY CARE DOCTORS AVERAGING AWVs on 8% to 33% of MEDICARE PATIENTS
Depending on where you are, most primary care physicians are not doing AWVs – in spite of the fact that since 2011, Medicare has been asking them to. Some primary care doctors have listened to STUPID advice and they’re not even taking Medicare patients. Hey – I can fix ignorance by educating folks – but I can’t fix stupid and it’s stupid to not take Medicare. Medicare is still the most PROFITABLE of all patients that a primary care doctor can see and I’ve proved it over and over and over when folks challenge me.
I ran some numbers that may surprise you:
- Lufkin TX 7 of the 21 FPs did AWVs
- Dallas TX 80 of the 259 FPs did AWVs
- Oklahome City 64 of the 235 FPs did AWVs
- Pensacola FL: 33 of the 90 FPs did AWVs
- Dayton, OH: 19 of the 94 FPs did AWVs
- Salt Lake City: 36 of the 150 FPs did AWVs
- San Diego CA: 55 of the 256 FPs did AWVs
- Nebraska: 234 of the 732 FPs did AWVs
- Nationwide: 28,961 of the 84,163 FPs did AWVs
