Intake Nurse Coordinator – Home Health

We are seeking a LPN for the position of Intake Nurse Coordinator. In this position, you will review and enter patient referral data received from inpatient facilities and community health providers. Through collaboration with referral sources and the Agency’s Business Development Team, you will facilitate the home health intake process. Responsibilities include insurance verification, obtaining service authorization, and triage and prioritization of patient referrals based on clinical need. You will also speak with patients and families to obtain pertinent information and address questions regarding VNA services. If you work effectively in a fast-paced environment, have keen attention to details, and excellent communication skills, this opportunity is just for you.
