In the latest segment of Mindful Connection's Conversations of Dementia, we had the privilege of sitting down with Shalini Patel, Care Transition Coordinator at St. Croix Hospice. With a profound dedication to hospice care, Shalini's journey through various healthcare roles has culminated in a position where she truly makes a difference in the lives of patients and their families.
Shalini's career in healthcare spans nearly 14 years. She began as a specimen processor in a lab and then assisted project managers in clinical trials. Realizing her passion for hands-on patient care, she earned her CNA license and an associate degree to become a physical therapist assistant. Her dedication and expertise led her to the role of Rehab Director, which she held for five years. However, her transition to St. Croix Hospice in January 2023 brought her the most fulfillment.
"Joining St. Croix Hospice felt like a blessing from God," Shalini reflects. "I was unsure about my next career step, and this role opened my eyes to the profound impact of hospice care. It's not just for the final days of life but a holistic approach that enhances quality of life."
Shalini underscores a pivotal point - the transformative power of early hospice care. "More than 75% of patients with life-limiting illnesses either start hospice in the final stages of illness or not at all," she explains. "Early hospice care is a beacon of hope, reducing medical costs, decreasing hospitalizations, and providing essential physical, emotional, and spiritual support. It allows patients to pass away in a place they call home, honoring their wishes." This early intervention can bring hope and comfort to patients and their families.
Shalini also underscores the comprehensive support available to families. "Families experience their journey alongside their loved ones, and hospice provides peace of mind knowing compassionate care is given," she says. At St. Croix Hospice, bereavement counselors support families for up to 13 months after a patient's passing. Support groups, spiritual guidance, and community resource identification are also available to families, ensuring they receive the care and support they need and are not alone in their journey. This comprehensive support system is designed to reassure and comfort families during a challenging time.
Shalini addresses a significant barrier: the mindset of "caring for our own" within minority communities. "Many minority populations are accustomed to caring for family members at home without external support, which can lead to caregiver burnout," she notes. "It's important to normalize asking for help and utilizing available resources. Medicare, Medicaid, and some private insurance cover hospice services. If there's no coverage, some hospices can work with families to ensure needed services get provided."

Reflecting on the importance of preparation, Shalini emphasizes that this empowers families and ensures they feel in control of their loved one's care:
"It's a journey, not a destination, and preparation is key. Discussing end-of-life wishes with loved ones is crucial. The pandemic highlighted the importance of having plans in place. St. Croix's Lighthouse Program helps families prepare, ensuring their wishes are honored."
Shalini's dedication to hospice care is a testament to her commitment to improving the quality of life for patients and their families. Her journey through the healthcare industry has equipped her with the knowledge and compassion needed to excel in her role at St. Croix Hospice.
