ELEVATOR Safety Plan
Purpose
The purpose of the Elevator Safety Program is to develop and implement the procedures for the identification, use, care, and maintenance of Elevators required to be used by visitors, tenants, and employees for the promotion of safety.
?All employees are required to follow the minimum procedures outlined in this program. Any deviations from this program must be immediately brought to the attention of the Program Administrator.

Any property managed by Property Management shall have a maintenance service agreement in place;

Program Responsibilities
Management. The management of Property Management is committed to the safety and health of its workers, tenants, and visitors. Management supports the efforts of the Elevator Safety Program Administrator. Management will regularly communicate with employees about this program. ?
Program Administrator. The Program Administrator which is in most cases the Property Manager reports directly to upper management and is responsible for the hazard assessments, implementation, training, and administration of the Elevator Safety program. The Program Administrator will monitor the results of the program to determine additional areas of focus as needed. The Program Administrator will also:

Property Management Responsibility
·????????Maintain the equipment in a safe operating condition and in conformance with the standards and codes adopted and keep the equipment free from reportable conditions;
·????????Property Manager will ensure maintenance or security staff is performing monthly Phase I and Phase II testing and documentation as outlined in this plan.
·????????Have an annual inspection of each unit of equipment to determine compliance with the applicable standards and codes adopted
·????????Obtain the services of an inspector registered with the department to perform inspections
·????????Have each unit of equipment tested to determine its safety and compliance with the requirements of the codes and standards.
·????????Ensure that all the tests required by the applicable standards and codes adopted in are performed; and have all violations cited on an inspection report:
·????????Corrected within 60 calendar days of the date of inspection;
·????????Have them under contract to be corrected and all work completed not later than the next inspection due date; or
·????????Have an approved waiver or delay.

·????????New equipment installations must have plans and other documents reviewed by the State regulating department and must be inspected and tested to determine their safety and compliance with the requirements of the codes and standards before being placed in service. The equipment must be free of any violations, unless a waiver, delay or new technology variance has been granted by the department in writing from the State, before being placed back into service.
·????????Altered equipment must have plans and other documents reviewed by the State regulating department and must be inspected and tested to determine its safety and compliance with the requirements of the codes and standards before being placed back in service. ·????????When regulated equipment is inspected:
·????????for the first time, the Property Manger must display the Proof of Inspection with a Proof of Inspection Sticker at the location defined by the State inside of the elevator until a current certificate of compliance is issued by the executive state director.
·????????On an annual basis, Property Management must display the most current certificate of compliance with a Proof of Inspection Sticker at a location inside the elevator, until a current certificate of compliance is issued by the executive director.
·????????The Property Manager must have equipment re-inspected and re-certified if:
·????????The equipment has been altered, including any alteration made to the interior of the elevator car enclosures or flooring;
·????????The equipment has been determined to have a reportable condition; or
·????????The inspection report shows an existing violation has continued longer than permitted in a delay granted by the executive director.

·????????The Property Manager must make available to the State department and all elevator personnel all maintenance and inspection records, maintenance control programs, maintenance, and repair manuals, and product specific inspection, testing, and maintenance procedures for each make and model number of the equipment as required by the applicable standards and codes
·????????The documents, records and programs required under subsection (a) must be in written or electronic format and be stored in the equipment room, machine room, machine space, control room, or control space or be accessible from a location within the building for that specific equipment.
·????????The Property Manager must have copies of all current department-issued waivers, delays, and new technology variances posted in the equipment room, machine room, machine space, control room, or control space in a readily accessible and visible location available to elevator personnel.

·????????The Property Manager must report all accidents involving equipment to the State department, using a department-approved form, within 24 hours of the accident.
·????????For any reported accident, the equipment shall be removed from service and shall not be moved (except as necessary to extricate an injured party), used, or returned to service until a representative of the State department completes an investigation and issues an approval to return the unit to service.
·????????If any equipment is determined to have a reportable condition by inspection or other means, the Property Manager must notify the State department in writing within 24 hours, and must place the unsafe equipment out of operation until repairs to correct the reportable condition(s) are completed. After repairs have been completed, the Property Manager must have the equipment re-inspected and re-certified and submit written verification to the department that the reportable condition has been corrected before returning the equipment to service.

Before any employee can perform testing and emergency response, they must first receive training in the form of a review of the Plan. Annual review of the Plan will include, at a minimum, the following subjects:

Written records will be kept which include trainee names, the type of training provided and the dates when training occurred. The Program Administrator will maintain these training records for 3 years.

The Program Administrator will maintain annual inspections, phase testing, and elevator maintenance documents for each elevator evaluated for 5 years.

At least annually, the Program Administrator will conduct a program review to assess the progress and success of the program. The review will consider the following:

Appendix A -Entrapment Procedure
Elevators are designed to protect passengers by stopping or limiting movement when the elevator control systems detect unsafe conditions or malfunctions. Elevator companies report that more persons are injured because of improper exiting from stalled elevators, than from any elevator malfunction. Because of this concern, the procedures outlined in this document will be followed whenever persons are unable to normally exit an elevator.
All Property Management Staff should be cognizant of the need of safety in all aspects of elevator operations. Elevator emergency phones are answered by the Elevator service company.?Elevator rescues should be performed by the Elevator service Company. During entrapment incidents, should any emergency develop, such as fire or emergency medical conditions for a passenger on the stalled elevator, the local fire department will be called for immediate response.
Types of Elevator Emergencies
The primary responsibility of RJL Property Management Operations team; Property Management, Engineers, or Security is to make certain that all safety considerations have been considered. This includes contacting the responding elevator service technicians, reassuring passengers, and assessing the situation for potential emergencies. The procedures are as follows;
1.??????If passengers are aboard a stalled elevator Contact Thyssenkrupp dispatch and technician and get and ETA. Thyssenkrup is available 24 hours a day 7 days a week. Note the time the call was made and the ETA of the tech If more than 30 minutes consider contacting the fire department.

2.??????If Onsite prior to the arrival of the Tech, go to the floor where the car is stopped. If possible, talk through the door and ask if everyone is ok.

3.??????NOTE: The phone inside the elevator is monitored by Thyssenkrup dispatch and they should keep an open line of communications with the people inside. Contact Thyssenkrup for communication assistance if an emergency persists.
?
4.??????If at any point they are experiencing any medical emergency call the fire department for an emergency extraction. This should try to be avoided unless the occupant is experiencing a medical emergency or extreme duress.
?
5.??????Complete Incident form for any emergency situations or building damages caused by first responders.
Appendix B - Elevator Monthly Testing of Phase I and II Fire Service
Procedure:
Phase I
Insert the key into the fire recall switch, typically located on the main access floor. Turn to the “ON” position. The elevator(s) should return to the landing where the key switch is located, and the doors should open. This is Phase I.
?Phase II
1. Locate the fire service key and insert the key into the fire service key switch inside the elevator.
2. Turn the key to the “HOLD” position and push an alternate “FLOOR SELECTION” button. The button should NOT light up.
3. Turn the key to the “ON” position. Push an alternate “FLOOR SELECTION” button. The button should now light up.
4. Push and hold the “DOOR CLOSE” button. If the elevator is equipped with an electronic reopening device, wave your hand in the door opening or between the doors. The doors should continue to close. When the doors are about halfway closed, release the “DOOR CLOSE” button and the doors should reverse direction and start to open. Before the doors are fully open, push and hold the “DOOR CLOSE” button again and the doors should reverse and close again. Hold the “DOOR CLOSE” button until the doors are completely closed. Release the “DOOR CLOSE” button and the doors should remain closed.
5. Push a “FLOOR SELECTION” button on an upper floor, and when the car starts in motion, push the “CALL CANCEL” button. The light for the “FLOOR SELECTION” button should turn off and the car should stop at the next available floor.
6. Push and hold the “DOOR OPEN” button and the doors should start to open. When the doors are about halfway open, release the “DOOR OPEN” button and the doors should reverse direction and close. Push and hold the “DOOR OPEN” button again until the doors reach the fully open position. Release the button and doors should stay in the open position.
7. Turn the key to the “OFF” position. The elevator doors should close, and the car should return to the main landing. Depending on the elevator’s code year, you may have to push the main landing “FLOOR SELECTION” button. At the main landing, push the “DOOR OPEN” button until the doors are fully open and then turn the key to the “OFF” position. Remove the key and insert it into the hall key switch.
8. Turn the key to the “RESET” or “BYPASS” position and then back to the “OFF” position. The elevator(s) should now be back in normal service.
9. Complete the Monthly Fire Service Test Log on the next page, which is a component of the Maintenance Control Program (MCP) document located in the machine room or electronically that must be viewable by the maintenance contractor, inspector and other elevator personnel.
