Edition 37: Advanced Decision Modeling Techniques in Healthcare
In this edition, we continue our exploration of decision modeling techniques from Pharmacoeconomics: From Theory to Practice, focusing on more advanced methods used in pharmacoeconomics, such as Markov models and sensitivity analysis.
What Are Markov Models?
While decision trees are useful for short-term or single-event decisions, Markov models are better suited for chronic diseases or long-term healthcare decisions where patients move between different health states over time. These models are ideal when the decision involves recurring events, such as relapses or the progression of chronic conditions.
How Do Markov Models Work?
In a Markov model, a patient can transition between various predefined health states. Each state has an associated cost and a probability of moving to another state. For example, a patient might move from "healthy" to "diseased" to "recovering" or "dead" over time. At each time interval (referred to as a Markov cycle), the model calculates the probability of remaining in or transitioning to a different health state.
A typical use of a Markov model in pharmacoeconomics might involve comparing the long-term costs and outcomes of two treatment strategies for a chronic disease. By running the model over many cycles, decision-makers can estimate the total cost and health benefits of each strategy over a patient’s lifetime.
Advantages of Markov Models
Markov models are particularly valuable for analyzing long-term treatments and chronic diseases because they account for changes in a patient’s health over time. They also allow healthcare professionals to model the impact of treatments on quality of life, costs, and survival rates over extended periods.
For instance, when treating conditions like heart disease or cancer, Markov models help assess not only the immediate cost and benefit of treatment but also how the patient’s health will evolve in the future, providing a comprehensive view of long-term outcomes.
Introduction to Sensitivity Analysis
One challenge in decision modeling is that real-world data are often uncertain. For example, the exact cost of a treatment or the probability of an adverse event might vary between patients or over time. Sensitivity analysis addresses this uncertainty by testing how changes in these variables affect the results.

Why Sensitivity Analysis Is Crucial
Without sensitivity analysis, decision models can give a false sense of precision. By testing how sensitive the results are to changes in key variables, healthcare decision-makers can identify which factors have the most impact on their decisions. For example, sensitivity analysis might reveal that the cost-effectiveness of a treatment is highly sensitive to the price of a drug but less sensitive to variations in treatment success rates.
In complex healthcare scenarios, this information is invaluable for making robust decisions that hold up under various conditions.
