Edition 11: Introduction to Cost-Utility Analysis (CUA)
In this edition and the next, we will be exploring Cost-Utility Analysis (CUA), an important tool in pharmacoeconomics for balancing both the quality and length of life. This summary is based on Chapter 6 of Essentials of Pharmacoeconomics, which explains how CUA helps healthcare professionals make decisions when both the effectiveness and the quality of life (QoL) associated with different treatments need to be considered.
What is CUA?
CUA is a type of economic evaluation that compares the cost of different interventions by measuring outcomes in terms of both quantity and quality of life. The most common outcome measure in CUA is the quality-adjusted life-year (QALY), which incorporates both mortality (years of life) and morbidity (the quality of those years).
When is CUA Useful?
CUA is particularly useful when interventions impact not only how long a patient will live but also the quality of that life. For instance, in cancer treatment, chemotherapy may extend life but comes with side effects that reduce the quality of those extra years. CUA helps healthcare professionals weigh these factors to determine the most appropriate treatment.

How Does CUA Differ from CEA?
Some consider CUA a subset of Cost-Effectiveness Analysis (CEA). However, the key difference is that CEA evaluates only natural health units, like life-years saved, while CUA incorporates patient preferences and utility measures, which reflect quality of life. This allows for a more nuanced evaluation of treatments that affect both lifespan and well-being.
In the next edition, we will discuss specific examples of how CUA is applied in real-world healthcare scenarios.
