Due to Faults/Identified by Design/Installation Audits/Resulting in Changes.

Generally the Design modifications resulting from a Design Audit which have Identified the need for a Re-Design leading to the replacement of equipment or making Design/Fabrication changes to Critical Frames/Supports such as the New Otis-LRT commissioned Platform for the Elephant & Castle Underground Station, for which Ron Cook took advice from me as an Independent Engineer Acting for Insurance/Assurance Agents. At that time I was Inspecting a 20 Tons Lifting Crane at a Marina in Walton On Thames, the Manager/Developper was a former Civil Engineer working Professionally in that Discipline. The Ropes of Lay-Construction similar to Traction Lifts Ropes, had a 20-30 degrees Dog Leg, my Advice to my Colleague from H. S. B. Haughtom who was in charge and whom I assited was to immediately Shut Down the Crane Lifting operations and Re-Rope (with New Drako or Equivalent), this was obviously Due to an Over-Load condition, which led to Stains beyond the Elastic Limit of the Rope Strands--Deformation of that extent is due to PLASTIC FLOW [Onset of]
  Returning to the telephone call by Ron Cook (Otis-LRT) who advised me then that the New Platform for Elephant & Castle had under LUL conducted tests deflected by 15 mm under a 500Kg point Load, positioned Eccentrically/Near the 4 Feet High Side Door to the Lift Car Enclosure, where in the event of a Trapping can connect the Stricken Lift to the adjoining one in the same "Circular" Lift Shaft/Well, this then Facilitates the transfer of Passengers in the Stricken Lift Cabin/Enclosure to the adjoining Lift so as to take the Passengers to a Safe Intermediate Area/Level or to Ticket Level.

In Hindsight after a Design Audit which identifies Risks/Dangerous Design Criteria, so as to implement and undertake Modifications/Redesigns . Design Audits reduce the incidence of Design Deficiencies and force the Manufacturer/Fabricator of the Component to Re-Design it , providing the requirement for a Design Audit is made at the outset and the implementation schedule the Auditor must be part of a team in which each member makes a distinctive improvement to the INSTALLATION.
