Don't Let Excess Cement Ruin Your Implant

I recently removed a cement retained implant restoration due to an insufficient mesial contact and this is what I found. The dental literature is full of articles describing the negative effects of cement extrusion on the health of the periodontium surrounding an implant (Wadhwani and Pineyro). I did not place this restoration but I'm sure some of my early work unfortunately has some cement extrusion. My goal is not to convince you to used cement or screw retained restorations, but rather how to use cement retained properly, if you so choose. Wadhwani and Pineyro describe a wonderful technique to help minimize excess cement. You essentially make a die of the intaglio restoration surface using a quick setting PVS material. This way, you can seat your crown on the PVS dye first, expressing the excess cement, and then seating on your abutment in the mouth.
